Introducción: La enfermedad de Alzheimer (EA), trastorno neurodegenerativo,
y por tanto, progresivo e irreversible, es la forma más frecuente de demencia;
biomarcadores plasmáticos afamina, IGHM, BACE1, cociente Aβ1-42/Aβ1-40, ptau181,

Introduction: Alzheimer’s disease (AD) is a progressive and irreversible brain
disorder and the most frequent form of dementia, a highly disabling syndrome.
However, this is the last stage of a neurodegenerative Continuum. Within this
biological framework, biomarkers are used to establish the definition of AD,
offering the possibility of an early diagnosis.
Objective: To assess the efficacy of blood biomarkers in the diagnosis of
sporadic AD during the predementia phase.
Methods: A systematic bibliographic search was carried out in PubMed and
Scopus. Afterwards, it was subsequently screened by applying inclusion and
exclusion criteria The most relevant data was grouped in a summary table. The
evidence levels of the Scottish Intercollegiate Guidelines Network were used to
assess the quality of the evidence and the Newcastle-Ottawa Scale and the
QUADAS-2 to assess the risk of bias.
Results: 11 studies published in the last 10 years were evaluated, collecting a
total of 6.555 participants. Plasma biomarkers afamine, IGHM, BACE1, ratio Aβ1-
42/Aβ1-40, p-tau181, GFAP, NFL, sAXL, IL-6 and YKL-40 revealed to be
associated with different AD features. Likewise, the majority showed potential
both in early diagnosis of AD and in the prediction of progression to dementia due
to AD.
Conclusions: Aβ1-42/Aβ1-40 ratio, p-tau181 and NFL are the plasma biomarkers
that have shown the greatest potential to be applied in clinical practice and to
detect patients with sporadic AD in the prodromal stage. [-]
