El tratamiento del cáncer de mama durante el embarazo. Una revisión integradora

Abstract

Introducción: Debido al aumento de los casos de cáncer de mama asociado al embarazo, es interesante conocer si es posible utilizar los tratamientos habituales para el cáncer de mama durante la gestación. Objetivos: Describir los diferentes tratamientos farmacológicos y quirúrgicos del cáncer de mama asociado al embarazo. Material y métodos: Se llevó a cabo una revisión integradora de la literatura a través de la búsqueda de artículos en las bases de datos de PubMed, BVS y La Biblioteca Cochrane Plus. Para ello se utilizaron términos clave procedentes de Descriptores en Ciencias de la Salud (DeCS) y Medical Subject Headings (MeSH). Resultados: Se han incluido en la revisión 32 artículos. Estos se han clasificado según base de datos, tipo de estudio, área geográfica, año de publicación, grupo de tratamiento y objetivo específico tratado. Discusión: Se han expuesto las indicaciones, complicaciones y ventajas del uso de la cirugía, radioterapia, quimioterapia, inmunoterapia y terapia endocrina, así como el uso de la lactancia materna durante o tras el empleo de estos tratamientos. Conclusión: De acuerdo con la literatura existente, se recomienda el uso de la cirugía y la quimioterapia durante el embarazo, principalmente a partir del segundo trimestre, sin embargo, la utilización de la radioterapia, inmunoterapia y terapia endocrina debe posponerse hasta después del parto para evitar complicaciones.

Introduction: Due to the increase in the number of cases of pregnancy-associated breast cancer, it is interesting to know if it is possible to use the usual treatments for breast cancer during pregnancy. Objective: To describe the different pharmacological and surgical treatments for pregnancyassociated breast cancer. Methodology: An integrative review of the literature was carried out by searching for articles in the databases of PubMed, BVS and The Cochrane Library Plus. Key terms from the Health Sciences Descriptors and Medical Subject Headings were used. Outcomes: 32 articles were included in the review. These were classified according to database, type of study, geographic area, year of publication, treatment group and specific objective treated. Discussion: The indications, complications, and advantages of the use of surgery, radiotherapy, chemotherapy, immunotherapy, and endocrine therapy, as well as breastfeeding after the use of these treatments have been presented. Conclusion: According to the existing literature, the use of surgery and chemotherapy during pregnancy is recommended mainly from the second trimester onwards; however, the use of radiotherapy, immunotherapy and endocrine therapy should be postponed until after delivery to avoid complications.
