INTRODUCCIÓN. Existen numerosos estudios sobre función pulmonar en niños
nacidos prematuros antes de los años 90, pero la evidencia es escasa para los adultos
nacidos prematuros en la era post-surfactante. El objetivo de este estudio fue estimar
la prevalencia de obstrucción bronquial y/o broncodilatación, y analizar su relación con
factores neonatales y asma.
MÉTODO. Estudio preliminar, observacional y transversal de 50 participantes. Todos
completaron el cuestionario ISAAC de asma y una espirometría forzada con prueba
broncodilatadora. Los parámetros espirométricos se midieron en porcentajes del
predicho y z-scores.
RESULTADOS. La prevalencia de broncodilatación en la muestra fue 12%, la de
asma 46%, y la de obstrucción varió en función del método de medición. La edad
gestacional media en el grupo con prueba broncodilatadora positiva (PBD+) fue
significativamente baja (28,35 ± 2,16 semanas). La proporción de PBD+ fue superior
en los expuestos a ventilación mecánica invasiva neonatal que en los no expuestos
(100% frente a 0%). Ningún paciente con PBD+ tuvo diagnóstico de asma. No se
encontró relación entre la broncodilatación y la displasia broncopulmonar, ni tampoco
entre la obstrucción bronquial y ninguna de las variables contrastadas.
CONCLUSIONES. La PBD+ parece ser el marcador más sensible y específico para la
detección de enfermedad pulmonar asociada a la prematuridad, pudiendo presentarse
como hallazgo único, sin la coexistencia de obstrucción bronquial ni de sintomatología
respiratoria o asma. [-]

INTRODUCTION. Most of the studies published deal with pulmonary function in infants
born preterm before the introduction of perinatal advances. New studies on long-term
prematurity consequences are required among adults born in the post-surfactant era.
The objective of this study was to estimate the prevalence of bronchial obstruction
and/or bronchodilator reversibility, and to analyze its relationship with neonatal factors
and asthma.
METHOD. Cross-sectional preliminary study of 50 participants. All of them completed
ISAAC questionnaire and underwent forced spirometry with bronchodilator testing. The
spirometric parameters were measured using percentage of predicted values and zscores.
RESULTS. The prevalence of bronchodilator response (BDR) in our sample was 12%
of asthma 46%, and about bronchial obstruction it varied depending on the
measurement method. The mean gestational age was significantly smaller in the group
with BDR (M: 28,35 ± 2,16 weeks). The proportion of BDR in those who required
neonatal invasive ventilation was significantly higher in comparison to those who were
not exposed (100% versus 0%). None of the patients with BDR had ever been
diagnosed with asthma or suffered from wheezing. We did not observe any statistically
significant association between bronchodilation and bronchopulmonary dysplasia,
active smoking, or hospitalization antecedents. We did not find statistically significant
results when it comes to the analysis between bronchial obstruction and any of the
contrasted variables.
CONCLUSIONS. BDR seems to be the most specific and sensitive marker in chronic
lung disease of prematurity, being able to appear as a single finding, without
coexistence of bronchial obstruction, respiratory symptoms, or asthma. [-]
