dc.description.abstract | ANTECEDENTS: El Sistema de París ha substituït al Sistema Papanicolaou com sistema diagnòstic de citologies d’orina. L’evidència actual fa avinent que el Sistema de París ha aconseguit augmentar la sensibilitat i el valor predictiu negatiu (VPN) sense perdre especificitat, i establir un risc de malignitat propi per a cada categoria diagnòstica. L’objectiu d’aquest estudi és conéixer els canvis que ha experimentat la sensibilitat, especificitat, valor predictiu positiu (VPP), VPN i el risc de malignitat en aquesta transició. METODOLOGIA: Avaluació de prova diagnòstica mitjançant una cohort retrospectiva a la qual es comparen dues sèries, una de 400 citologies per al Sistema Papanicolaou i l’altra de 400 citologies al Sistema de París. RESULTATS: Per a la detecció de Carcinoma Urotelial d’Alt Grau descrivim al Sistema de París millor especificitat (93.82% vs. 98.64%; χ 2 =11.825, p-valor=.001) i VPP (70.6.% vs. 39.5%; χ 2 =4.550, p-valor=.044), sense canvis significatius a la sensibilitat (37.5% vs. 53.5% χ 2 =1.558, p-valor=.299) i VPN (94.8% vs. 96.4%; χ 2 =1.169, p-valor=.183). El risc de malignitat experimenta un canvi estadísticament significatiu per a la categoria atípia (40.0% vs. 1.6%; χ 2 =20.282, p-valor=.001), mentre la resta de categories romanen sense canvis estadísticament significatius. CONCLUSIÓ: El Sistema de París ha aconseguit millorar l’especificitat i el valor predictiu positiu i estableix un risc de malignitat propi per cada categoria, permetent fonamentar un seguiment clínic específic per cada resultat. | ca_CA |
dc.description.abstract | BACKGROUND: Paris System has replaced Papanicolaou System as reference for reporting urinary cytology. Nowadays it is believed that the Paris System has improved the sensibility, the negative predictive value (NPV) without losing specificity, and it has established the risk of malignancy for each cytological category. The aim of this study is to determine the changes on sensibility, specificity, positive predictive value, NPV and risk of malignancy in this transition. METHODS: Evaluation of the diagnostic power of urinary cytology through retrospective cohort comparing two series, one composed of 400 cytologies for Papanicolaou System and another composed of 400 cytologies for Paris System. RESULTS: Paris System has got better specificity (93.82% versus 98.64%; χ 2 =11.825, p-value=.001) and PPV (70.6% versus 39.5%; χ 2 =4.550, p-value=.044) without changes in sensibility (37.5% versus 53.5% χ 2 =1.558, p-value=.299) and NPV (94.8% versus 96.4%; χ 2 =1.169, p-value=.183) for the detection of High-Grade Urothelial Carcinoma. The risk of malignancy for the atypical category increases from low to high levels (40.0% versus 1.6%; χ 2 =20.282, p-value=.001), keeping the other categories without significant statistical changes. CONCLUSION: Paris System improves specificity and positive predictive value, and establishes a better risk of malignancy for each category, which will allow the clinician to determine a specific management for each result. | ca_CA |
