Protocolo asistencial sobre le manejo y prevención de las fracturas por fragilidad

Abstract

La osteoporosis es una enfermedad esquelética crónica caracterizada por una disminución generalizada de la resistencia ósea. Esta alteración predispone a un mayor riesgo de fracturas por fragilidad, que, a su vez, se asocian a un incremento de la morbimortalidad y del riesgo de refractura, con repercusión tanto a nivel clínico, social como económico. En España, la prevalencia de osteoporosis es del 5.4%, y la incidencia de fracturas por fragilidad y refracturas está en aumento debido al incremento de la esperanza de vida de la población. Por todo ello, es fundamental realizar una correcta prevención secundaria de fracturas por fragilidad en pacientes fracturados osteoporóticos. El presente Trabajo de Fin de Grado ha consistido en la elaboración de un protocolo de actuación para el Servicio de Reumatología del Hospital General Universitario de Castellón (HGUCS), cuyo objetivo ha sido estandarizar el manejo del paciente fracturado osteoporótico y, adaptarlo a aquellos pacientes cuyo centro de referencia es el Hospital General Universitario de Castellón. Para asegurar un correcto programa de prevención secundaria de fracturas por fragilidad, y así disminuir el número de refracturas, se ha planteado la apertura de una Unidad de Fractura por Fragilidad o FLS (Fracture Liaison Service) en el Hospital General Universitario de Castellón. Para todo ello, se ha llevado a cabo una búsqueda sistemática de la evidencia científica más actualizada sobre el tema y se han formulado las recomendaciones a partir de estas evidencias.

The osteoporosis is a chronic skeletal disease characterized by a general decrease of the bone resistence. This alteration predisposes to a higher fragility fractures risk, which are associated to an increase in the morbidity and mortality and the risk of refracture, with repercussions both on a clinical, social and economic level. In Spain, the prevalence of the osteoporosis is 5.4% and the incidence of the fragility fractures and refractures is increasing due to the population life expectancy increase. Therefore, it is essential to carry out a correct secondary prevention of fragility fractures in osteoporotic fractured patients. The present Final Degree Project has consisted in the development of an assistance protocol for the Reumatology Service of the General University Hospital of Castellón or HGUCS (Hospital General Universitario de Castellón), whose objective is to standardize the management of the osteoporotic fractured patient and adapt it to those patients whose referral center is the General University Hospital of Castellón. To ensure a correct secondary prevention program of fragility fractures, and thus reduce the number of refractures, the opening of Fracture Liaison Service or FLS has been proposed in the General University Hospital of Castellón. For all of this, a systematic research of the most upadated scientifc evidence on this subject has been carried out and recommendations have been formulated based on these evidences.
