Introducción: Actualmente, el cáncer de cérvix es el cuarto cáncer en frecuencia en la
mujer a nivel mundial, por ello la necesidad de elaborar una guía clínica con respecto a
la estandarización de su manejo. En función del estadio de la enfermedad se encuentran
diversas alternativas para su abordaje teniendo además en cuenta las diferentes
situaciones especiales que pueden surgir. Sin embargo, no existe una guía óptima que
estandarice el manejo de esta patología o lo exponga de manera clara y concisa, de ahí
la necesidad de elaborar una guía adecuada para el manejo del cáncer de cérvix. El
grueso de este trabajo ha sido elaborado bajo las bases de dos guías existentes: NCCN
y ESGO; y en menor medida de la SEGO. Concretamente, la oncoguía de la SEGO
queda desactualizada pues no incluye datos precisos sobre los diferentes estadios de
enfermedad así como aquellos referidos al seguimiento. La guía de la NCCN es muy
completa pero debido a dicha complejidad puede resultar confusa su interpretación así
como la extracción rápida de datos. Por último, destaca la lucidez expositiva de la ESGO
pero no profundiza en contenido clínico.
Objetivos: Realizar una guía asistencial en la práctica clínica que oriente a la hora de
tomar decisiones en referencia al manejo de las pacientes con sospecha o diagnóstico
de cáncer de cérvix en el Departamento de Salud de Castellón y estandarice las
opciones terapéuticas establecidas en la bibliografía previa.
Metodología: Durante los meses de octubre, noviembre y diciembre del año 2020 se
realizó una revisión sistemática de la bibliografía existente hasta la fecha según criterios
de evidencia científica: fundamentalmente basándose en guías clínicas como la NCCN
(The National Comprehensive Cancer Network), ESGO (The European Society of
Gynaecological Oncology); respaldadas por la agencia de búsqueda médica NIH
(National Institutes of Health), y en menor medida de otras guías como la AIOM
(Associazione Italiana di Oncologia Medica), SEGO (Sociedad Española de Ginecología
y Obstetricia), BGCS (British Gynaecological Cancer Society) y SEOM (Sociedad
Española de Oncología Médica).
Resultados: El manejo de la enfermedad se clasifica en función de los diferentes
estadios, especificando las recomendaciones generales a seguir así como diversas
explicaciones y aclaraciones que hacen de la guía una herramienta entendible y extrapolable a cualquier servicio de oncología ginecológica. Aunque la última palabra
siempre la tendrá la paciente y serán determinantes sus deseos.
Conclusión: Tanto esta guía como las guías en las cuales se ha basado este
trabajo,son revisiones sistemáticas, elaboradas por expertos y por tanto, recomendadas
por ellos. Consisten en valoraciones generales sobre el manejo de los tumores de cérvix
incluyendo el diagnóstico, tratamiento y segumiento. [-]

Introduction: Currently, cervical cancer is the fourth most common cancer in women
worldwide, hence the need to develop clinical guidelines for standardised management.
Depending on the stage of the disease, there are various alternatives for its approach,
taking into account the different special situations that may arise. However, there is no
optimal guideline that standardises the management of this pathology or sets it out in a
clear and concise manner, hence the need to develop an appropriate guideline for the
management of cervical cancer. The bulk of this work has been developed on the basis
of two existing guidelines: NCCN and ESGO; and to a lesser extent from the SEGO.
Specifically, the SEGO oncoguide is outdated, as it dates from 2018 and does not include
precise data on the follow-up of the disease. The NCCN guideline is very complete but
due to its complexity it can be confusing to interpret and to extract data quickly. Finally,
the ESGO is clear in its exposition but does not go into clinical content.
Aim: To create a clinical practice guideline to advise decision-making in the management
of patients with suspected or diagnosed cervical cancer in the Castellón Health
Department and to standardise the therapeutic options established in previous literature.
Methods: During the months of October, November and December 2020, a systematic
review of the existing literature to date was carried out according to scientific evidence
criteria: Primarily based on clinical guidelines such as the NCCN (The National
Comprehensive Cancer Network), ESGO (The European Society of Gynaecological
Oncology); endorsed by the medical search agency NIH (National Institutes of Health),
and to a lesser extent from other guidelines such as AIOM (Associazione Italiana di
Oncologia Medica), SEGO (Spanish Society of Gynaecology and Obstetrics), BGCS
(British Gynaecological Cancer Society) and SEOM (Spanish Society of Medical
Oncology).
Results: The management of the disease is classified according to the different stages,
specifying the general recommendations to be followed as well as various explanations
and clarifications that make the guide an understandable tool that can be extrapolated
to any gynaecological oncology department. Although the patient will always have the
last word and her wishes will be decisive.
Conclusions: Both this guideline and the guidelines on which this paper is based are
systematic reviews, prepared by experts and therefore recommended by them. They
consist of general assessments on the management of cervical tumours including
diagnosis, treatment and follow-up. [-]
