Estudio de tendencias en el uso de terapias biológicas en la artritis reumatoide en el Área de Salut de Castellón

Abstract

Introducción: La artritis reumatoide (AR) es una enfermedad autoinmune crónica de etiología desconocida que cursa con afectación articular y sistémica. Está caracterizada principalmente por la inflamación articular, artralgias y destrucción de la membrana sinovial de las articulaciones. El tratamiento más actual se realiza con terapias biológicas que suponen un avance clínico pero un coste económico elevado. Objetivo: Analizar la tendencia en el uso de las terapias biológicas para el tratamiento de la artritis reumatoide, el coste de estas y la optimización practicada en los últimos 20 años en Castellón. Método: Se realizó un estudio observacional, descriptivo y retrospectivo. Trabajamos con una muestra de 310 pacientes del servicio de reumatología del HGUCS diagnosticados de AR en tratamiento con TB. Resultados: El uso de las terapias biológicas ha sido cada año una práctica más extendida, inicialmente los anti-TNF fueron los fármacos más utilizados, pero con la aparición de nuevas opciones el grupo de otros biológicos ha supuesto la tendencia más habitual. El coste de la TB ha ido creciendo anualmente en este periodo, sumando en total un coste de 16.430.934,64€, tendencia contraria a la que ha sufrido el coste medio anual por dispensación que ha ido disminuyendo. Conclusiones: El 36% de los pacientes que iniciaron una TB continuaban en tratamiento al final del seguimiento. Del total de los pacientes tratados durante los últimos 6 años, un 24% optimizó su dosis, lo que supuso un ahorro de 515.119€, es decir un ahorro del 11,86% de lo que habría sido el coste total previsto por ficha técnica.

Introduction: Rheumatoid arthritis (RA) is a chronic autoimmune disease of unknown etiology with joint and systemic involvement. It is primarily characterized by joint inflammation, arthralgias, and destruction of the synovial membrane of the joints. Current treatments with biological therapies are clinically effective but at high economic cost. Objective: To analyze the trend in the use of biological therapies for the treatment of rheumatoid arthritis, its cost and optimization practices in the last 20 years in Castellón. Method: An observational, descriptive and retrospective study was conducted. We worked with a sample of 310 patients from the rheumatology unit at HGUCS diagnosed with RA in treatment with BT. Results: The use of biological therapies has become more widespread every year. Initially, antiTNFs were the most widely used drugs, but with the appearance of new options the group of other biologics has become the most common trend. The cost of BT has increased annually over the last 20 years, reaching total values of 16,430,934.64€, a trend that is contrary to the average annual cost per dispensation, which has been decreasing. Conclusions: 36% of patients who started a BT finished the follow up with it. Of those treated during the last 6 years, 24% optimized their dose, thus representing a saving of 515,119€, meaning 11.86% of what would have been the total standard cost.
