Objetivo: Estudiar la utilidad de la punción aspiración por aguja fina (PAAF), la biopsia con aguja
gruesa (BAG) y de ambas técnicas conjuntamente para obtener el diagnóstico de nódulos pulmonares
sin contacto pleural, tomando las muestras guiadas por tomografía computarizada (TC), así como las
complicaciones asociadas que pudiesen aparecer tras estos procedimientos.
Material y métodos: Se realizaron un total de 182 biopsias de pulmón que fueron clasificadas en
tres grupos en función del método empleado y del material obtenido: PAAF (Grupo 1, n = 85),
BAG (Grupo 2, n = 15) y aquellas en las que se empleó tanto la PAAF como la BAG
(Grupo 3, n = 82). Se calculó la precisión diagnóstica de cada grupo, comprobando el número de
diagnósticos certeros que aportaban y si existía una relación estadísticamente significativa entre grupo
y porcentaje de diagnósticos correctos. Por último, realizamos un análisis descriptivo de las
características principales de los datos, incluyendo el análisis molecular.
Resultados: La técnica con la que se consiguió un mayor porcentaje de diagnósticos correctos fue
mediante el método combinado PAAF y BAG, alcanzando un 96,34% de aciertos mientras que la
menos resolutiva fue la PAAF, con un 81,18% de aciertos. Con la BAG se obtuvo un 86,67% de
diagnósticos correctos. No existe una relación estadísticamente significativa entre la realización de un
método (PAAF, BAG o PAAF y BAG) y la obtención de un diagnóstico correcto, la presencia de
neumotórax, ni un aumento en los días de hospitalización pero sí existe esta relación con la colocación
de un tubo de drenaje pleural para el tratamiento del neumotórax.
Conclusiones: La técnica que ha obtenido un porcentaje de aciertos mayor ha sido mediante el uso
de PAAF y BAG pese a no haber encontrado una relación estadísticamente significativa (p < 0,05)
entre ambas variables. Sí que existe esta relación con la necesidad de colocación de un tubo de drenaje
pleural, que fue menor al realizar PAAF más BAG. [-]

Objective: To study the diagnostic accuracy of fine needle aspiration puncture (FNA), coarse needle
biopsy (CNB) and both techniques together to obtain the diagnosis of pulmonary nodules without
pleural contact, taking the samples guided by computed tomography (CT), as well as the associated
complications that could appear after these procedures.
Material and methods: A total of 182 lung biopsies were performed. Depending on the method
used and the material obtained, these biopsies were classified into three groups: FNA
(Group 1, n = 85), CNB (Group 2, n = 15 ) and those in which both FNA and CNB are used
(Group 3, n = 82). The diagnostic accuracy of each group is calculated by checking the number of
accurate diagnoses that they provide and whether there was a statistical significance between group
and percentage of correct diagnoses. Finally, we perform a descriptive analysis of the main
characteristics of the data, including molecular analysis.
Results: The technique that achieves a higher percentage of correct diagnoses was combining FNA
with CNB, reaching a 96,34% of successes while the least decisive was the FNA by itself, with
81,18% of successes. With CNB, 86,67% of correct diagnoses were obtained. There is no statistical
significance between using a method (FNA, CNB or both FNA and CNB) and obtaining a correct
diagnosis, the presence of pneumothorax and an increase in hospitalisation days but this statistical
significance does exist with the placement of a pleural drainage tube for the treatment of
pneumothorax.
Conclusions: The technique that has achieved a higher percentage of correct diagnoses was
combining FNA with CNB although no statistical significance (p < 0.05) was found between both
variables. There is a statistical significance between belonging to a group of biopsy sampling
methodology with the need to place a pleural drainage tube, which was lower when performing FNA
plus CNB. [-]
