INTRODUCTION: Studies show that the laparoscopic (VL) route is as safe as the
open route (VA) for the repair of incisional hernias. Our objective is to demonstrate that
there is a bias in the selection of patients regarding the size of eventration and the
safety of both routes for these sizes.
MATERIAL AND METHODS: Population of 194 patients operated between May
2013 and May 2019. Operated by the same surgeon with the same antibiotic
prophylaxis, general and preoperative anaesthesia, with a minimum follow-up of one
year. The means have been compared with t-Student. SPSS Statistics has been used
for methodological analysis. The bibliographic review has been in Pubmed and
Cochrane and the journals Cirugía Española and Cirugía Andaluza.
RESULTS: 120 patients operated on by VA and 74 by VL. The size averages are
6.07x7.59 cm (46.07 cm2) for the VL and 9.67x12.37 cm (119 cm2) for the VA. The
mode of the sizes is 5x5cm in the VL and 10x10cm in the VA. 22 complications
(11.34%) have arisen between both groups. Immediate 1 VL haemorrhage, 1 VA
visceral injury and 11 VA surgical site infections have occurred. Delayed, 4 seromas
and 4 recurrences due to VA and 1 recurrence due to VL.
DISCUSSION: Small and medium-sized incisional hernias are operated on by VL,
while large ones are operated on by VA. Given this bias, both repair pathways are safe
for these eventration sizes. [-]
