Elección de la vía de acceso para la reparación de las hernias incisionales en función de su tamaño

Abstract

INTRODUCCIÓN: Estudios demuestran que la vía laparoscópica (VL) es igual de segura que la vía abierta (VA) para la reparación de hernias incisionales. Nuestro objetivo es demostrar que existe un sesgo en la selección de pacientes en cuanto al tamaño de eventración y la seguridad de ambas vías para esos tamaños. MATERIAL Y MÉTODOS: Población de 194 pacientes operados entre mayo de 2013 y mayo de 2019. Intervenidos por el mismo cirujano con misma profilaxis antibiótica, anestesia general y preoperatorio, con seguimiento mínimo de un año. Las medias se han comparados con t-Student. Se ha utilizado el SPSS Statistics para el análisis metodológico. La revisión bibliográfica ha sido en Pubmed y Cochrane y las revistas Cirugía Española y Cirugía Andaluza. RESULTADOS: 120 pacientes operados por VA y 74 por VL. Las medias de tamaño son de 6.07x7.59 cm (46.07 cm2) para la VL y de 9.67x12.37 cm (119 cm2) para la VA. La moda de los tamaños es de 5x5cm en la VL y 10x10cm en la VA. Han surgido 22 complicaciones (11.34%) entre ambos grupos. Inmediatas se han producido 1 hemorragia por VL, 1 lesión de víscera por VA y 11 infecciones del sitio quirúrgico por VA. Tardías, 4 seromas y 4 recidivas por VA y 1 recidiva por VL. DISCUSIÓN: Hernias incisionales de pequeño y mediano tamaño son operadas por VL mientras que las de gran tamaño lo son por VA. Teniendo en cuenta este sesgo, ambas vías de reparación son seguras para estos tamaños de eventración.

Operated by the same surgeon with the same antibiotic prophylaxis, general and preoperative anaesthesia, with a minimum follow-up of one year. The means have been compared with t-Student. RESULTS: 120 patients operated on by VA and 74 by VL. The size averages are 6.07x7.59 cm (46.07 cm2) for the VL and 9.67x12.37 cm (119 cm2) for the VA. The mode of the sizes is 5x5cm in the VL and 10x10cm in the VA. 22 complications (11.34%) have arisen between both groups. Immediate 1 VL haemorrhage, 1 VA visceral injury and 11 VA surgical site infections have occurred. Delayed, 4 seromas and 4 recurrences due to VA and 1 recurrence due to VL. DISCUSSION: Small and medium-sized incisional hernias are operated on by VL, while large ones are operated on by VA. Given this bias, both repair pathways are safe for these eventration sizes.
