Introducción: Las descripciones de cambios vasculares a nivel cerebral en las
enfermedades neurodegenerativas han dado pie a la hipótesis neurovascular, que teoriza
una posible influencia de los factores de riesgo cardiovascular en la etiopatogenia de estas
enfermedades.
Objetivos: Determinar la influencia de los factores de riesgo cardiovascular en la
aparición y progresión de la enfermedad de Alzheimer y la enfermedad de Parkinson, así
como dilucidar si el tratamiento de dichos factores de riesgo consigue ralentizar su
avance.
Métodos: Se realizó una búsqueda bibliográfica en Scopus y PubMed que posteriormente
fue cribada de acuerdo a los criterios de inclusión/exclusión previamente especificados.
Para evaluar la calidad de la evidencia se emplearon los niveles de evidencia del Scottish
Intercollegiate Guidelines Network, y para valorar los sesgos la Newcastle-Ottawa Scale
y la RoB 2 tool.
Resultados: Se incluyeron 24 estudios (20 de cohortes, 3 de casos y controles y un ensayo
clínico) con un tamaño muestral variable entre 83 y 2,6 millones de participantes. Tres
reportaron asociaciones inversas del Parkinson con la actividad física, tres con el consumo
de café y cuatro con el tabaco. Cuatro encontraron relaciones positivas entre IMC y
Alzheimer, y tres efectos beneficiosos de fármacos en la reducción del riesgo de estas
enfermedades. [-]

Introduction: The description of cerebral vascular changes in neurodegenerative
diseases has generated the neurovascular hypothesis, that suggests that cardiovascular
risk factors may take part in neurovascular diseases’ aetiology.
Objectives: To assess the influence that cardiovascular risk factors have in the onset and
progression of Alzheimer’s disease and Parkinson’s disease and to determine if the
treatment of those risk factors slows down their progression.
Methods: A bibliographic search was performed on Scopus and PubMed. Afterwards,
the results of that process were revised and the articles that did not fit the pre-specified
inclusion/exclusion criteria were discarded. The evidence levels of the Scottish
Intercollegiate Guidelines Network were used in order to assess the quality of the
evidence, and the Newcastle-Ottawa Scale and the RoB 2 tool in order to assess the risk
of bias.
Results: 24 publications were included in this systematic review (20 cohort studies, 3
case-control studies and one controlled clinical trial) with a number of participants
ranging between 83 and 2,6 million people. Three publications showed reverse
associations between Parkinson’s disease and physical activity, another three with coffee
consumption and four more with smoking. Four publications showed positive
associations between BMI and Alzheimer’s disease, and another three beneficial effects
of medication on the reduction of these diseases’ risk.
Conclusions: Obesity and metabolic health are associated with less Alzheimer’s disease
risk, and smoking, physical activity and tea and coffee consumption are protective against
Parkinson’s disease. ARBs (and their combination with certain statins), metformin and
DPP-4 inhibitors reduce the risk of Alzheimer’s disease. [-]
