dc.description.abstract | Diseño del estudio: Observacional, descriptivo, transversal y retrospectivo. Introducción: El hallux valgus (HV) se define como una desviación lateral y rotación grave del primer dedo del pie, con desviación medial del primer metatarsiano y la aparición de una prominencia ósea medial denominada bunion. Su corrección supone uno de los principales retos dentro de la cirugía del antepié. La osteotomía proximal de apertura (OPA) en el metatarsiano es una técnica quirúrgica indicada en deformidades moderadas-graves, pero no exenta de complicaciones, por lo que en los últimos años está siendo sustituida por otras técnicas menos exigentes. Objetivos: Analizar las complicaciones asociadas a la técnica OPA con fijación mediante placa para la cirugía de hallux valgus en pacientes intervenidos en el Hospital Provincial de Castellón en los últimos 10 años, comparando con la literatura para respaldar o no su indicación. Material y métodos: Se recogieron datos clínicos y radiológicos de 34 pacientes, que se analizaron estadísticamente mediante el programa Statistical Package for the Social Sciences 28 (IBM SPSS Inc, Chicago, IL). Resultados: Todos los individuos presentaron alguna complicación, como ausencia o pérdida de corrección radiológica (64’7% y 29’4%), recidiva (38’2%), problemas de partes blandas (47’1%), alargamiento metatarsal (64’7%), rigidez o artrosis metatarsofalángica (2’9% y 41’2%), molestias por el material de osteosíntesis (30’4%) y dolor residual (32’4%). No se demostró ninguna relación estadísticamente significativa entre las variables comparadas. Conclusión: En nuestro estudio, la recidiva, pérdida de corrección, infección, artrosis, y molestias del material de osteosíntesis mostraron peores resultados en comparación con la literatura. Fue destacable la curación clínica, y los mejores resultados en cuanto al alargamiento metatarsiano, rigidez y dolor residual. Todo ello podría explicar la sobreindicación de esta técnica en este centro. | ca_CA |
dc.description.abstract | Study design: Observational, descriptive, cross-sectional and retrospective. Introduction: Hallux valgus (HV) is defined as a severe lateral deviation and rotation of the big toe, with medial deviation of the first metatarsal and the appearance of a medial bony prominence called bunion. Its correction is one of the main challenges in forefoot surgery. The proximal opening osteotomy (PAO) in the metatarsal is indicated in moderate-severe deformities but, as it is not without complications, in recent years it has been replaced by other less demanding techniques. Objectives: To analyze the complications associated with the PAO technique with plate fixation for hallux valgus surgery in patients operated on at the Provincial Hospital of Castellón during the last 10 years, comparing it with the scientific literature whether to support or not its indication. Material and methods: Clinical and radiological data were collected from 34 patients, which were statistically analyzed using the Statistical Package for the Social Sciences 28 program (IBM SPSS Inc, Chicago, IL). Results: All individuals presented some complication such as absence or loss of radiological correction (64’7% and 29’4%), relapse (38’2%), soft tissue problems (47’1%), metatarsal lengthening (64’7%), stiffness or osteoarthritis metatarsophalangeal joint (2’9% and 41’2%), discomfort due to the osteosynthesis material (3’4%) and residual pain (32’4%). No statistically significant relationship was demonstrated between the variables compared. Conclusion: Relapse, loss of correction, infection, osteoarthritis, and osteosynthesis material discomfort showed worse results compared to the literature. Clinical healing was remarkable, and the best results in terms of metatarsal lengthening, stiffness and residual pain too. All this could explain the overindication of this technique in this center. | ca_CA |
