Introduction:
Atrial fibrillation (AF) is the most common sustained tachyarrhythmia and is
associated with an increased risk of thromboembolism. AF is asymptomatic in 90% of
the cases, making its diagnosis difficult, although it is detectable through
cardiostimulation cardiac devices, which allow remote monitoring (RM).
Aims:
The main objective of the study is to determine the subclinical AF survival curve in
patients with implantable cardiac devices included in RM, by nursing.
Methodology:
In this observational study, all patients included in RM were included, with no prior
history of AF before entering the remote system and that presented at least an episode
of subclinical AF in the follow-up by nursing (n=260). Continuous data were
compared using Student’s test or Mann-Whitney U test; categorical data were
compared by the χ2 test or Fisher’s exact test. The Kaplan-Meier method was used to
measure the survival curve. Independent factors associated with subclinical AF were
estimated with Cox proportional hazards model.
Findings:
The cumulative incidence of subclinical AF was 13% (n=33). In the follow-up, AF
was statistically associated with age (OR 1.04, 95% CI 1.01-1.08, p=0.044); mortality
(OR 3.08, 95% CI 1.42-6.68, p=0.040); and betablocker treatment (OR 0.46, 95% CI
0.23-0.92, p=0.025). The nurse referred 31 patients (94%) to the face-to-face
consultation for therapeutic interventions. Total time mean of follow-up was 829 ±
447 days, time from subclinical AF episode to nurse diagnosis was 21±27 days and
the time from detection to medical consultation was 12±12 days.
Conclusions:
The cumulative incidence of subclinical AF was approximately 13%. RM allowed an
early diagnosis, which eased the individualized therapeutic measures. All this could20
have positive repercussions in the prognosis. RM coordinated by nursing as a member
of the multidisciplinary team is an effective and achievable reality. [-]

La fibrilación auricular (FA) es la taquiarritmia auricular sostenida más frecuente y
asociada a mayor riesgo de episodios tromboembólicos. La FA es asintomática en un
90% de los casos, dificultándose su diagnóstico, aunque es detectable mediante
dispositivos cardiacos de cardioestimulación, que permiten monitorización remota
(MR).
Objetivos:
El objetivo principal de este estudio es determinar la curva de supervivencia de FA
subclínica en pacientes portadores de dispositivos cardiacos incluidos en MR, por parte
de enfermería.
Metodología:
Estudio observacional retrospectivo. Se incluyeron a todos los pacientes con MR, sin
antecedentes de FA previos a su incorporación al sistema y que mostraron algún
episodio de FA subclínica durante el seguimiento de enfermería (n=260). La
comparación de variables continuas se realizó mediante el test de Student o test de
Mann-Whitney; las categóricas se compararon mediante la prueba de la Chi2 o test
exacto de Fisher. El análisis de supervivencia se realizó mediante la curva de KaplanMeier. Los factores independientes asociados a FA subclínica se estimaron mediante
regresión de Cox.
Resultados:
La incidencia acumulada de FA subclínica fue de 13% (n=33). La FA en el
seguimiento se asoció de forma significativa con la edad con un HR de 1,04 [IC 95%
(1,01-1,08); p=0,044], la mortalidad con un HR de 3,08 [IC 95% (1,42-6,68); p=0,040]
y la toma de tratamiento betabloqueante con un HR de 0,46 [IC 95% (0,23-0,92);
p=0,025]. La enfermera derivó a la consulta a 31 pacientes (94%) para intervención
terapéutica. La media total de tiempo de seguimiento fue de 829 ± 447 días, el tiempo
desde que se produjo el episodio de FA subclínica hasta que la enfermera lo
diagnosticó fue de 21 ± 27 días y el tiempo desde la detección hasta consulta médica
fue de 12 ± 12 días.18
Conclusiones:
La incidencia acumulada de FA subclínica rozó el 13%. La MR permitió un
diagnóstico precoz, que facilitó acciones terapéuticas individualizadas. Todo ello
podría tener repercusiones positivas en el pronóstico. La MR coordinada por
enfermería como parte de un equipo multidisciplinar es una realidad factible y efectiva. [-]
