dc.description.abstract | Fatigue is one of the most disabling symptoms of Multiple Sclerosis (MS), and greatly affects the quality of life of patients. Knowing the causes of fatigue is complicated due to the multitude of factors involved, the problems in its definition and the dif ficulties in obtaining objective measurements. These problems together with the lack of proven scientific research are the reasons why the specific cerebral bases of fatigue have not yet been well defined, which would help us to better understand the disea se and to develop more effective treatments better adapted to the needs of these patients. The aim of this review has been to analyze the scientific literature that has studied fatigue using magnetic resonance (MR), in order to synthesize the existing info rmation and find the points in common of the different investigations. The results indicate that injuries at the regional level are more significant than at a global level. At the structur al level, gray matter atrophy ( G M ) of frontal and parietal regions, as well as basal ganglia and thalam us, and white matter atrophy (WM ) of the corpus callosum stand out. At the functional level, these same regio ns appear altered in the research , however there are some discrepancies in the functional connectivity (F C ) of t hese regions that the researchers have not been able to explain satisfactorily. The conclusion of this work is that in spite of the advances it is necessary to continue investigating to know more in detail how they correlate the different factors involved and to deepen in the structure - function cerebral relationship to create a model capable of explaining the processes involved in fatigue. | ca_CA |
dc.description.abstract | La fatiga es uno de los síntomas más incapacitantes de la Esclerosis Múltiple (EM) , y afecta en gran medida la calidad de vida de los pacientes. Conocer las causas de la fatiga es complicado debido a la multitud de factores implicados , los problemas para su definición y las dificultades para conseguir mediciones objetivas. Estos problem as junto con la falta de investigación científica contrastada son los motivos por los que aún no han sido bien definidas las bases cerebrales específicas de la fatiga , las cuales nos ayudaría n a comprender mejor la EM y a desarrollar tratamientos más efica ces y mejor adaptados a las necesidades de est os pacientes. Esta revisión tiene como objetivo analizar la literatura científica que ha estudiado la fatiga usando resonancia magnética (RM) , con el fin de sintetizar l a información existente y de hallar los puntos en común de las diferentes investigaciones. Los resultados analizados indican que las lesiones a nivel regional son más significativas que a nivel global . A nivel estructural destaca la atrofia en sustancia gris ( SG ) de regiones front ales y parietales, así como ganglios basales y tálamo, y la atrofia en sustancia blanca ( SB ) del cuerpo calloso. A nivel funcional estas mismas regi ones aparecen alteradas en los estudio s, sin embargo existen algunas discrepancias en la conectividad funcio nal (CF) de dichas regiones que los investigadores no han con seguido explicar de forma satisfactoria. La conclusión de este trabajo es que a pesar de los avances e s necesario seguir investigando para conocer más en detalle como correlacionan los distintos factores implicados y profundizar en la relación cerebral estructura - función para crear un modelo capaz de explicar los procesos implicados en la fatiga . | ca_CA |
