Cirugía mínimamente invasiva de columna en fracturas toracolumbares : revisión bibliográfica

Abstract

¿Cuándo debemos operar una fractura traumática toracolumbar mediante cirugía mínimamente invasiva? Objetivos: El principal objetivo de esta revisión es estudiar el tratamiento de diferentes fracturas toracolumbares de columna mediante cirugía mínimamente invasiva, comparándola con la cirugía abierta o el tratamiento conservador. Material y métodos: Consiste en una revisión sistemática de artículos procedentes de las principales bases de datos: Pubmed, Cochrane, Tripdatabase y Embase, seleccionando artículos publicados en los últimos diez años relacionados con las fracturas traumáticas de la columna toracolumbar y basados en la cirugía mínimamente invasiva, convencional o abierta y/o tratamiento conservador, siendo catalogadas mediante las clasificaciones AOs y/o TLICS. Resultados: Los artículos analizados reflejan que no hay diferencias estadísticamente significativas en las variables radiológicas estudiadas entre la cirugía mínimamente invasiva de columna y la cirugía convencional o tratamiento conservador para las fracturas toracolumbares. No obstante, para las variables clínicas se han encontrado diferencias significativas a favor de MIS. Conclusiones: Sigue sin establecerse un consenso sobre el tipo de cirugía a realizar respecto a cuál trata de mejor forma una fractura toracolumbar. Sin embargo, la MIS nos aporta ciertos beneficios en determinadas variables clínicas.

When should we operate a traumatic thoracolumbar fracture by minimal invasive surgery? Objectives: The main goal of this review is to analyse the treatment of the different thoracolumbar spine fractures by minimal invasive surgery, comparing it with other types of surgical methods. Methods and materials: The following final degree project consists of a bibliographic review of articles from the main database: Pubmed, Cochrane, Tripdatabase and Embase selecting some clinical essays, cohorts, systematic revisions or published methanalysis in the last ten years. These are closely related to the traumatic fractures of the thoracolumbar spine and based on minimal invasive/open surgery or conservative treatment, in addition to the articles that mention the AOs and/or TLICS’ classifications. Results: The analysed articles show that there are not significant differences in the radiologic variables between the minimal invasive surgery and the open surgery or conservative treatment for the thoracolumbar spine fractures. However, we have found significant differences in the clinical variables in favour of the minimal invasive surgery. Conclusions: In the light of the results, there is no establishment of a consensus on the type of surgery to be performed, in order to treat the best way possible a thoracolumbar fracture. However, we can obtain benefits from minimal invasive surgery in some clinics variables.
