Efectos del metilfenidato en el rendimiento lector de niños con TDAH

Abstract

Stimulant medication is the most widely used treatment for the attention deficit hyperactivity disorder (ADHD). Its effectiveness on behavior and performance on cognitive tasks has been demonstrated, but the evidence of its positive effects on specific skills is still scarce. The purpose of this study was to determine the effects of methylphenidate on different reading components in children with ADHD. The participants were 13 children with ages ranging from 7 to 12 years and IQs of 80 or more. A battery of tests was applied to rate lexical access and comprehension of stories at two points in time, without methylphenidate and after receiving 0.5 milligrams/kilogram of the psycho-stimulant. The pre-posttreatment intragroup analyses revealed significant improvements in the post-treatment condition only on some lexical access tasks, specifically word reading time and pseudo-homophones. With the methylphenidate, a significant increase was found in the quality of the recalled propositions belonging to the causal network, main introduction, secondary introduction, and events presented in the stories. The findings indicate that the methylphenidate improves some aspects of reading, possibly those most related to attention and sustained effort

La medicación estimulante es el tratamiento del trastorno por déficit de atención con hiperactividad (TDAH) más extendido. Su efectividad sobre el comportamiento y el rendimiento en tareas cognitivas ha sido demostrada, pero la evidencia de sus efectos positivos en aprendizajes instrumentales aún es escasa. El propósito de esta investigación fue determinar los efectos del metilfenidato sobre diferentes componentes de la lectura en niños con TDAH. Participaron 13 niños con edades entre 7 y 12 años y CI igual o superior a 80. Se aplicó una batería de tests para valorar el acceso al léxico y la comprensión de historias en dos momentos, sin metilfenidato y tras recibir 0.5 miligramo/kilogramo del psicoestimulante. Los análisis intragrupo pre-postratamiento revelaron mejoras significativa en la condición de post-tratamiento sólo en algunas tareas del acceso al léxico, concretamente en el tiempo de lectura de palabras y pseudohomófonos. También se observó, con el metilfenidato, un incremento significativo en la calidad de las proposiciones recordadas pertenecientes a la red causal, introducción mayor, introducción menor, y sucesos presentados en los cuentos. Los hallazgos aportan cierta evidencia de que el metilfenidato mejora algunos aspectos de la lectura, posiblemente los que tienen más relación con la atención y el esfuerzo sostenido.
