Introducción: El control de la temperatura dirigido a través de hipotermia es una de las
intervenciones que parecer ser eficaz para prevenir un mayor daño cerebral y reducir el riesgo
de lesión tisular entre pacientes resucitados de un paro cardíaco. No obstante, los datos
disponibles sobre su efectividad siguen permaneciendo discutidos.
Objetivo: Determinar el efecto de la aplicación de hipotermia terapéutica sobre una mejor
evolución del paciente tras recuperación de una parada cardiorrespiratoria.
Metodología: Se realizó una búsqueda bibliográfica en las bases de datos PubMed, Cochrane
Library, SciELO, Scopus y ProQuest. Los descriptores estandarizados obtenidos de los tesauros
MeSH y DeCS a partir de las palabras clave se combinaron mediante los operadores booleanos
“AND” y “OR” y se aplicaron los filtros correspondientes a cada base de datos para cumplir
con los criterios de selección establecidos. La evaluación de la calidad metodológica se llevó a
cabo mediante la herramienta CASPe.
Resultados: Se incluyeron un total de once artículos, correspondientes a revisiones sistemáticas
y metaanálisis publicados entre 2015 y 2021. Atendiendo especialmente a año de publicación,
tipo de estudio y nivel de calidad metodológica, se exponen las características principales de
cada artículo.
Conclusiones: La evidencia de apoyo es de poca certeza como para recomendar una
temperatura de 32-36 °C en pacientes que permanecen comatosos después de una parada
cardiorrespiratoria. Se recomienda el control continuo de la temperatura central y la prevención
de la fiebre, aunque se necesita más investigación. [-]

Background: Targeted temperature management through hypothermia is one of the
interventions that appears to be effective in preventing further brain damage and reducing the
risk of tissue injury among patients resuscitated from cardiac arrest. However, the available
data on its effectiveness remain disputed.
Aim: To determine the effect of the application of therapeutic hypothermia on a better evolution
of the patient after recovery from cardiorespiratory arrest.
Methods: A bibliographic search was carried out in the databases PubMed, Cochrane Library,
SciELO, Scopus and ProQuest. The standardised descriptors obtained from the MeSH and
DeCS thesaurus from the keywords were combined using the boolean operators "AND" and
"OR" and the corresponding filters were applied to each database to meet the established
selection criteria. The methodological quality assessment was made using the CASPe tool.
Results: A total of eleven articles were included, corresponding to systematic reviews and
meta-analysis published between 2015 and 2021. The main characteristics of each article are
presented, paying special attention to the year of publication, type of study and level of
methodological quality.
Conclusions: The supporting evidence is of low certainty to recommend a temperature of 32-
36°C in patients who remain comatose after cardiorespiratory arrest. Continuous monitoring of
core temperature and prevention of fever is recommended, although more research is needed. [-]
