INTRODUCCIÓN: La aparición de los biomarcadores ha creado un nuevo concepto de
la enfermedad de Alzheimer, en el que se intentan caracterizar los principales procesos
patológicos de la EA décadas antes de que aparezcan los primeros síntomas clínicos,
por lo que suponen un paso hacia el cambio de diagnóstico y de futuras posibilidades
de tratamiento.
OBJETIVOS: Resumir los biomarcadores que se encuentran actualmente en uso clínico
y los que se están desarrollando para detectar la enfermedad de Alzheimer en sus
diferentes etapas.
METODOLOGÍA: Tras una revisión exhaustiva de la bibliografía en Pubmed y Cochrane
Library, se incluyeron los artículos que cumplieran los criterios de inclusión. Se elaboró
una tabla con la extracción de datos y se evalúo la calidad metodológica mediante las
escalas SIGN.
RESULTADOS: Se obtienen 17 estudios de los cuales 14 son meta-análisis. Se
encuentran diferencias significativas en la mayoría de los biomarcadores estudiados
entre pacientes diagnosticados con enfermedad de Alzheimer y los controles, tanto en
líquido cefalorraquídeo como en sangre, sobre todo cuando se realiza un análisis a nivel
grupal.
CONCLUSIÓN: Los marcadores descritos podrían usarse como biomarcadores no solo
para detectar la enfermedad de Alzheimer cuando los síntomas son claros, sino también
en la etapa temprana, ya que tales procesos inflamatorios pueden preceder a los
síntomas de demencia y detectarse antes. [-]

INTRODUCTION: Biomarkers have created a new concept of Alzheimer's disease, in
which the main pathological processes of AD have already occurred decades before we
can clinically diagnose the first symptoms. Therefore they represent a step towards the
change of diagnosis and future treatment possibilities.
OBJECTIVES: To summarize the biomarkers that are currently in clinical use and those
that are being developed to detect Alzheimer's disease in its different stages.
METHODOLOGY: After an exhaustive review of the literature found in Pubmed and
Cochrane Library, articles that met the inclusion criteria were admitted. A table was
prepared with the data extraction and the methodological quality was evaluated using
the SIGN scales.
RESULTS: 17 studies were obtained, of which 14 are meta-analysis. Significant
differences are found in most of the biomarkers studied between patients diagnosed with
Alzheimer's disease and controls, both in cerebrospinal fluid and in blood, especially if
they are studied at the group level.
CONCLUSION: The markers described could be used as biomarkers not only to detect
Alzheimer's disease when the symptoms are clear, but also in the early stage, since
inflammatory processes can precede dementia symptoms and be detected earlier. [-]
