INTRODUCCIÓN Y OBJETIVOS: La leucemia mieloide aguda (LMA) es una neoplasia
caracterizada por la proliferación descontrolada de precursores hematopoyéticos
mieloides. Predomina en mayores, con peor estado funcional y escasa respuesta
terapéutica. El objetivo del presente trabajo es describir la población de LMA en mayores
de 65 años diagnosticados y seguidos en el Departamento de Salud de Castellón desde
1994 hasta 2020.
MATERIAL Y MÉTODOS: Es un estudio observacional descriptivo retrospectivo. Para
el diagnóstico se siguieron criterios de la clasificación FAB y de la OMS (2008-2016).
Se empleó la clasificación ELN para dividir los grupos pronósticos citogenéticos. Se hizo
un estudio estadístico descriptivo y de supervivencia con curvas de Kaplan Meier y se
analizó por grupos de tratamiento y períodos de tiempo del estudio.
RESULTADOS: Se diagnosticaron 215 pacientes cuya edad media fue 74,7 años. Un
59,1% presentó comorbilidades y un 27,4% LMA secundaria. Clínicamente,
predominaron anemia y síntomas constitucionales. 1/3 no tenía estudio genético y un
66,7% presentó riesgo citogenético intermedio. Un 38,6% recibió tratamiento paliativo,
un 32,1% quimioterapia intensiva y un 7,5% hipometilantes (HMA). La supervivencia
mediana global fue 2, 12, y 11 meses, respectivamente. Los resultados no han cambiado
con el paso del tiempo.
CONCLUSIÓN: Los pacientes mayores presentan características clínicas, biológicas y
moleculares asociadas a peor pronóstico. La quimioterapia intensiva aporta más
supervivencia, pero no es apta para muchos enfermos y deben emplearse terapias
menos agresivas, cuyos resultados son decepcionantes. Únicamente se observa una
leve mejoría con HMA, que puede ser la base para una mejora futura. [-]

INTRODUCTION AND OBJECTIVES: Acute myeloid leukemia (AML) is a neoplasm
characterized by uncontrolled proliferation of myeloid hematopoietic precursors. It
predominates in the elderly, with worse performance status and poor therapeutic
response. The aim of this study is to describe the AML population in patients over 65
years of age diagnosed and followed up in the Castellón Health Department from 1994
to 2020.
MATERIAL AND METHODS: This is a retrospective descriptive observational study.
FAB and WHO (2008-2016) classification criteria were followed for diagnosis. The ELN
classification was used to divide the cytogenetic prognostic groups. A descriptive
statistical study was performed, survival with Kaplan Meier curves and analyzed by
treatment groups and time periods of the study.
RESULTS: A total of 215 patients were diagnosed with a mean age of 74.7 years. A
59.1% presented comorbidities and 27.4% secondary AML. Clinically, anemia and
constitutional symptoms were predominated. 1/3 had no genetic study and 66.7%
presented intermediate cytogenetic risk. 38.6% received palliative treatment, 32.1%
intensive chemotherapy and 7.5% hypomethylating agents (HMA). Median overall
survival was 2, 12, and 11 months, respectively. The results have not changed over time.
CONCLUSION: Older patients have clinical, biological and molecular characteristics
associated with worse prognosis. Intensive chemotherapy provides better survival, but it
is not suitable for many patients and less aggressive therapies must be used, which
results are disappointing. Only a slight improvement is observed with HMA, which may
be the basis for future improvement. [-]
