Vasculitis asociada a anca con afectación renal: cambios histológicos y su valor pronóstico.

Abstract

Introduction : Systemic vasculitis is a clinical - pathological process characterized by inflammation and vascular necrosis. T he objective of this study is to analyze which renal histological parameters are involved in a greater affectation of renal function. Material and methods : A retrospective analysis of patients diagnosed with systemic vasculitis between 2011 and 2016 was ca rried out. Twenty - three patients with a histological diagnosis concordant with ANCA + vasculitis was selected. Results : Of the general data that were obtained: the average age of debut was 64 ± 13 years. The most frequent signs and symptoms were: hematuria and proteinuria; constitutional syndrome, arthromyalgia and pulmonary clinic, respectively. Regarding the ANCA analys is by SI, there was a higher prevalence in the PANCA. The histological results were classified according to the Berden process: Focal (15.4%), crescentic (26.1%), mixed (34.8%) and sclerotic (30.4%). The crescent ic and sclerotic subgroups, showed major det erioration of renal function both at the beginning of the diagnosis and during follow - up; more likely to require hemodialysis. The death was related to the mixed and crescentic subgroups. Discussion: the percentage of healthy glomeruli is the best predicto r of re nal function. Berden's classification system explains the relationship with the evolutionary prognosis of the disease.

Introducción : La vasculitis sist é mica es un proceso cl í nico - patol ó gico caracterizado por la inflamaci ó n y necrosis vascular. El objetivo de este estudio es analizar que parámetros histológicos renales están im plicados en una mayor afectación de la función renal. Material y métodos: Se realiz ó un análisis retrospectivo de pacientes diagnosticados de vasculitis sistémica entre 2011 y 2016. Se seleccionaron 23 pacientes con un diagnóstico histológico concordante con vasculitis ANCA+. Resultados : De los datos generales que se obtuvieron : la edad media debut fue 64 ± 13 años. Los signos y síntomas más frecuentes fueron: hematuria y proteinuria; síndrome constitucional, artromialgia y clínica pulmonar, respectivamente. En cuanto al análisis ANCA por IF hubo mayor prevalencia en los PANCA. Los resultados histológicos se clasificaron según el proceso de Berden: Focal (15.4%), crescéntica (26.1%), mixta (34.8%)y esclerótico (30.4%). Los subgrupos crescénti co y esclerótica, presentaron mayor deterioro de la función renal tanto al inicio del diagnóstico, como durante el seguimiento; con mayor tendencia a requerir hemodiálisis. El e xitus se relacionó con subgrupos mixta y crescéntica . Discu sión: el porcentaje de glomérulos sanos es el mejor predictor de la función renal. El sistema de clasificación de Berden explica la relación con el pronóstico evolutivo de la enfermedad.
