dc.description.abstract | Atrial fibrillation (A F) is the most frequent cardiac arrhythmia, estimating a prevalence of 1 - 2% that increases with the age. Its main complication is the risk of stroke and other embolic episodes, which is 5 times higher compared with the general population. Although vitamin K antagonists (VKA) are effective in reducing this risk, there are certain inconveniences such as the need of a monitoring, frequent dose adjustment, numerous pharmacological and food interactions and, as a consequence, an increased risk of thrombosis and hemorrhage. The appearance of new direct oral anticoagulants (NOAC) based on the inhibition of thrombin or Xa factor considerably extends the therapeutic options. This study has consisted in the realization of an assistance protocol for the internal medici ne / cardiology S ervices of La Plana Hospital. This protocol has the objective of standardize an adequate anticoagulant management of non - valvular AF in patients whose health area is that of this hospital. For that, a systematic search of the current scien tific evidence has been carried out, to then apply it for our health department area. This protocol addresses the therapeutic arsenal that we currently have, based on the embolic and hemorrhagic risk of each patient as well as their complications and strat egies for an adequate treatment. A reduced version of the protocol has also been created. | ca_CA |
dc.description.abstract | La fibrilación auricular (FA) es la arrit mia cardíaca sostenida más frecuente, estimándose una pre valencia de l 1 - 2% que aumenta con la edad . Su principal complica ción es el riesgo de ictus y otros episodios tromboembólicos incrementándo se en 5 veces a l de la población general . Si bien los antagonistas de la vitamina K (AVK) son eficaces para reducir este riesgo , existen ciertos inconvenientes como la necesidad de monitorización, ajuste frecuente de dosis, numerosas interacciones farmacológicas y ali menticias y, como consecuencia, mayor riesgo de trombosis y hem orragia . La aparición de nuevos anticoagulantes orales directos (NACO) basados en la inhib ición de trombina o el factor Xa a mplía considerablemente las opciones terapéuticas . El pr esente TFG ha consistido en la realización de un protocolo asistencial para el Servicio de Medicina Interna y Cardiología del Hospital La Plana. Dicho protocolo tiene el objetivo de estandarizar un adecuado man ejo anticoagulante de la FA no valvular en pacientes cuya área de salud sea la de este hospital. Para ello, se ha realizado una búsqueda sistemática de la evidencia científica actual , para después aplicarla a nuestro departamento de sal ud. S e aborda el arsenal terapéutico del que disponemos actualmente basa do en el riesgo tromboembólico y hemorrágico de cada paciente así como sus complicaciones y estrategias para un adecuado tratamiento .
