JUSTIFICACIÓN Y OBJETIVOS: La donación de productos sanguíneos es en un gesto altruista de mucha importancia. La transfusión de hemoderivados es un recurso terapéutico y en ocasiones preventivo, de vital importancia. Dada la actual escasez de dichos productos y el constante aumento de la complejidad asistencial, este trabajo tiene como fin el conocer cómo, cuanto y porqué se transfunde en el Hospital General Universitario de Castellón, con el objetivo de tener información que pueda mejorar el uso racional de estos hemoderivados.
METODOLOGÍA: Para llevar a cabo este trabajo descriptivo retrospectivo, se realizó una base de datos anónima con el programa Microsoft Excel 2013, con la información procedente del sistema informático BLUE Blood Unit Expert 5.2® que utiliza el Banco de Sangre del centro, y que incluía grupos sanguíneos de donante y receptor, nivel de hemoglobina, servicio transfusor, grado de urgencia y escrutinio de anticuerpos irregulares entre otras variables. Se seleccionaron algunas de las variables cualitativas y cuantitativas que se analizaron mediante porcentajes sobre el total y mediante medias y desviación estándar respectivamente. Se incluyeron todas las transfusiones de las que se tenía constancia en la base de datos BLUE® versión 5.2. en el período comprendido entre el 1 de julio y 31 de diciembre de 2016
RESULTADOS: En el periodo de estudio se transfundieron un total de 3725 unidades de concentrado de hematíes. Además de examinar el grado de cumplimentación de las hojas de petición transfusional y de la base de datos, el análisis nos permitió conocer la distribución de la transfusión por grupos sanguíneos de paciente y de hematíes transfundidos, el grado de compatibilidad donante-receptor, el grado de urgencia de la transfusión y el resultado de escrutinio de anticuerpos irregulares. También pudimos conocer los servicios más transfusores: Hematología y Hemoterapia (21,23 %), Cirugía y Anestesia (14,68%), Urgencias (13,69%) y UCI (8,51%). Además, se comprobó que el nivel medio de hemoglobina a la hora de transfundir fue de 7,71 ± 0,95 g/dL, y pudimos comprobar la tendencia transfusional de cada Servicio por cifra de hemoglobina. CONCLUSIÓN: Con el presente estudio se comprobó que los servicios del HGUCS siguen las recomendaciones que hacen las diferentes guías como la SETS (Sociedad Española de Transfusiones Sanguineas) o la AABB (American Asociation of Blood Banks). Nos ha permitido además tener un dibujo del perfil real de la transfusión de hematíes en el hospital (grupos sanguíneos, servicios transfusores, grado de urgencia, escrutinios). Por último, se descubrieron algunos aspectos mejorables en la cumplimentación de las solicitudes como el nivel de hemoglobina y el diagnóstico que podrían mejorar aún más la información. Este trabajo presentó como principal limitación, además de la completa cumplimentación de las hojas de petición, el conocimiento de datos clínicos concretos que complementaran la información obtenida de las hojas transfusionales. JUSTIFICATION AND OBJECTIVES: The donation of blood products is an altruistic gesture of great importance. The transfusion of blood products is a therapeutic and sometimes preventive resource, of vital importance. Due to the current shortage of these products and the constant increase in the complexity of care, this work aim is to know how, how much and why is transfused in the HGUCS, with the objective of having information that can improve the rational use of these blood products.
METHODOLOGY: In order to carry out this retrospective descriptive work, an anonymous database was made with the program Microsoft Excel 2013, with information from the BLUE Blood Unit Expert 5.2® computer system used by the Center Blood Bank, which included Donor and recipient blood groups, hemoglobin level, transfusion service, degree of urgency, and scrutiny of irregular antibodies among other variables. We selected some of the qualitative and quantitative variables that were analyzed by percentages over the total and by mean and standard deviation respectively. All confirmed transfusions were included in the BLUE® version 5.2 database in the period from 1 July to 31 December 2016.
RESULTS: A total of 3725 units of packed red blood cells were transfused during the study period. In addition to examining the degree of completion of the transfusion request sheets and the database, the analysis allowed us to know the distribution of transfusion by blood groups of transfused patients and packed red blood cells, degree of donor-recipient compatibility, degree of urgency of the transfusion and the result of scrutiny of irregular antibodies. The most transfusive services wer Hematology and Hemotherapy (21.23%), Surgery and Anesthesia (14.68%), ER (13.69%) and ICU (8.51%). In addition, it was found that the average level of hemoglobin at the time of transfusion was 7.71 ± 0.95 g / dL, and we were able to verify the transfusional tendency of each service per hemoglobin figure.
CONCLUSION: The present study showed that the HGUCS services follow the recommendations made by the different guides such as SETS (Spanish Society of Blood Transfusions) or AABB (American Association of Blood Banks). It has also allowed us to have a drawing of the actual profile of the transfusion of red blood cells in the hospital (blood groups, transfusion services, degree of urgency, counts). Finally, some improvement aspects were found in completing applications such as hemoglobin level and diagnosis that could further improve the information. In addition to completing the request sheets, this paper presents the knowledge of specific clinical data that complement the information obtained from the transfusion sheets. [-]
