Objective: To determine which vascular dermoscopic structures are associated with a scar free from tumor recurrence and which are suggestive of recurrence of basal cell carcinoma (BCC). Methods: An amount of 29 biopsies were performed in the Hospital General Universitario de Castellón and Hospital de Vinaròs between January 2011 and December 2016. They were divided in two groups, one formed by the scars with tumor recurrence, where 17 of the biopsies were included and another with the scars without tumor recurrence, formed by the 12 remaining biopsies. All of them included data on age, sex, tumor location, tumor recurrence, chronic solar photoexposure, scar maturation time, primary tumor from which the scar came and visible dermoscopic structures. Results: The presence of arborizing telangiectasias is 58.8% in the recurrences and are absent in the scars. The short-fine telangiectasias (17.6% in recurrences and 33.3% in scars) and linear-irregular vessels (11.8% in recurrences and 50% in scars) are present in both groups in a similar way. The coma, hairpin, dotted and glomerular vessels appear in the scars without recurrence only (8.3%, 25%, 16.7% and 8.3%, respectively).
Conclusions: The presence of arborizing vessels in a CBC scar suggests recurrence. It appears that there is no specific dermoscopic vascular structure of normal scarring. The presence of coma, dotted, hairpin and glomerular vessels is not associated with recurrence. [-]
