Pla de cures estandaritzat per a l'adherència terapèutica del pacient esquizofrènic

Abstract

L’esquizofrènia és un trastorn mental, que es caracteritza per les distorsions de la percepció, el pensament i les emocions i en la que són freqüents les al·lucinacions, sobretot auditives. Com és una malaltia de llarga durada, els pacients i els familiars han de ser conscients de la importància del compliment terapèutic. Per tant la manca d’adherència al tractament pot tindre conseqüències negatives, com les recaigudes que provoquen majors tasses d’hospitalització i per tant, un augment dels costos sanitaris. Aquesta adherència terapèutica es pot aconseguir amb un correcte seguiment per part de tot el equip multidisciplinari. Per aquest motiu és convenient l’elaboració d’un Pla de Cures Estandaritzat que permeta als professionals d’infermeria prestar cures d’una forma racional i sistemàtica. Aquest Pla de Cures està realitzat segons el Procés d’Atenció d’Infermeria (PAI), seguint el mètode científic aplicat a la pràctica assistencial. En el Pla de Cures Estandaritzat per a millorar l’adherència en els pacients amb esquizofrènia, la valoració està basada en els Patrons Funcionals de la Salud de M. Gordon i les intervencions d’infermeria s’engloben en 3 àrees: l’establiment d’una relació terapèutica entre professional d’infermeria-pacient, activitats educatives i de modificació de la conducta i finalment la informació al pacient i a la família sobre el seu corresponent tractament. En definitiva, el principal resultat d’aquest Pla de Cures seria la consecució de l’adherència terapèutica al tractament. Aleshores per a que aquest Pla de Cures Estandaritzat resulte efectiu, convindria posar-lo en pràctica en les Àrees de Salut Mental.

Schizophrenia is a mental disorder characterized by distortions of perception, thinking and emotions in which there are frequent hallucinations, especially auditory hallucinations ones. As a long-term illness, patients and families should be aware of the importance of adherence to therapy. Therefore the lack of compliance to treatment can have negative consequences, such as relapses that cause higher rates of hospitalization and for that reason increased healthcare costs. This therapeutic adherence can be achieved with a correct monitoring by the entire multidisciplinary team. For this reason, it’s convenient to develop a Care Plan allowing nurses to provide some care in a rational and systematic form. This Standardized Care Plan is made according to the Nursing Care Process, following the scientific method applied to clinical practice. In the Standardized Care Plan to improve adherence in patients with schizophrenia, the assessment is based on the Functional Health Patterns by M. Gordon and nursing interventions fall into three areas: establishing a therapeutic relationship between nurse-patient, educational activities and behaviour modification and finally information to the patient and the family about the appropriate treatment. In short, the main result of this Care Plan would be to achieve adherence to treatment. So for this Standardized Care Plan resulting effective it would be convenient to put it into practice in the areas of Mental Health.
