dc.description.abstract | Antecedentes: el asma es una inflamación crónica y heterogénea de las vías respiratorias. Engloba al endotipo T2 y al no T2, siendo el T2 predominante, por lo que su conocimiento fisiopatogénico y desarrollo terapéutico es mucho mayor. Esta situación, sumada al desconocimiento de los mecanismos exactos que guían la inflamación no T2 y su respuesta pobre a tratamientos tradicionales, deriva en un asma generalmente grave y difícil de tratar. Objetivo: revisar estudios que evalúen fármacos con dianas terapéuticas propias del asma grave no T2, bien sean biológicos o no biológicos. Así como revisar la utilidad de estos fármacos en el tratamiento del endotipo no T2. Metodología: se revisaron 11 ensayos clínicos. Fueron seleccionados según los criterios de elegibilidad, tras una búsqueda bibliográfica en PubMed, Cochrane Library y Scopus. Luego se confeccionó una tabla de extracción de datos con los hallazgos principales. Finalmente, se realizó un análisis del riesgo de sesgo y evaluación de la calidad de la evidencia. Resultados: las terapias biológicas anti-TNFα, anti-IL-17, anti-IL-23, anti-IL-8 y anti-FLAP, podrían no ser útiles en el tratamiento del asma no T2. En cambio, la azitromicina y terapias anti-IL1 y anti-TSLP sí demostraron su posible utilidad como tratamiento. No se pudo declarar la seguridad de fármacos anti-IL-17/antiIL-13, por riesgo de consecuencias inmunogénicas. Conclusión: pese al desarrollo de fármacos efectivos, no existe una terapia establecida para el asma T2 bajo. Por ello es vital investigar las vías inflamatorias del endotipo no T2, para descubrir nuevas dianas terapéuticas y, por ende, medidas farmacológicas más efectivas. | ca_CA |
dc.description.abstract | Backround: asthma is a chronic and heterogeneous inflammation of the airway. It encompasses the T2 and non-T2 endotypes. T2 is predominant therefore the physiopathogenic knowledge and therapeutic development of T2 is much greater. This situation, added to the lack of knowledge of the exact mechanisms that guide non-T2 inflammation and a poor response to traditional treatments, results in an asthma generally severe and difficult to treat. Objective: review studies that evaluate drugs with therapeutic targets typical of non-T2 severe asthma, whether biological or non-biological. As well as review the usefulness of these drugs in the treatment of non-T2 endotype. Methods: 11 clinical trials were reviewed. They were selected according to the eligibility criteria after a bibliographic search in the PubMed, Cochrane Library and Scopus databases. Then a data extraction table was made with the main findings. Finally, a risk of bias analysis and an assessment of the quality of the evidence were performed. Results: anti-TNFα, anti-IL-17, anti-IL-23, anti-IL-8, and anti-FLAP biologic therapies may not be useful in the treatment of non-T2 asthma. On the other hand, azithromycin, anti-IL1 and anti-TSLP therapies showed their possible usefulness as treatment. The safety of anti-IL-17/anti-IL-13 drugs cannot be declared due to the risk of immunogenic consequences. Conclusion: despite the development of effective drugs there is no established therapy for low T2 asthma. For this reason it is vital to investigate the inflammatory pathways of the non-T2 endotype in order to discover new therapeutic targets and, therefore, more effective pharmacological measures. | ca_CA |
