INTRODUCCIÓN: los pacientes politraumatizados son una preocupación global y requieren de una evaluación precoz y precisa para identificar lesiones intraabdominales y hemorragias potencialmente mortales. La EcoFast es rápida, sencilla, no invasiva, pero es observador dependiente, se usa en pacientes hemodinámicamente inestables. El TC por su parte es muy sensible y específico, y se utiliza en pacientes estables. El objetivo del trabajo es comprobar si se cumple el protocolo y la sensibilidad, especificidad, VPP y VPN de la EcoFast.
MÉTODOS: se realiza un estudio analítico, observacional, longitudinal y retrospectivo. Se incluyen 39 pacientes con politraumatismos a los que se les ha realizado EcoFast y/o TC al llegar al Hospital General de Castellón durante un periodo de 3 años (2019-2022). Se realiza un análisis descriptivo de las variables, el chi-cuadrado con una significación estadística de un p valor < 0,05, y se calcula la sensibilidad, especificidad, VPP y VPN de la EcoFast.
RESULTADOS: 30 de los pacientes se encuentran estables y a 32 se les ha realizado una EcoFast, siendo negativo en 29 individuos y en 7 no se ha realizado. A 31 pacientes se les ha realizado un TC, siendo patológico en 25, no concluyente en 2, y en 9 no se ha realizado. Se ha concluido que no hay dependencia entre el resultado de la EcoFast y si se les ha realizado un TC. La sensibilidad es 10%, la especificidad 100%, el VPP 100% y el VPN 10%.
CONCLUSIONES: la EcoFast es una prueba con una sensibilidad pobre, una elevada especificidad, alto VPP y bajo VPN. Por ello, un resultado negativo en la prueba no descarta lesiones, y se debe comprobar con otra prueba, como un TC en pacientes estables. [-]

INTRODUCTION: Polytraumatized patients are a global concern and require early and accurate evaluation to identify intra-abdominal injuries and potentially life-threatening hemorrhages. The EcoFast is a fast, simple, and non-invasive technique, but it is observer-dependent and used in hemodynamically unstable patients. On the other hand, a CT is highly sensitive and specific and is used in stable patients. The aim of this study is to determine protocol adherence and assess the sensitivity, specificity, PPV, and NPV of EcoFast.
METHODS: An analytical, observational, longitudinal, and retrospective study is being conducted. It includes 39 patients with polytrauma who undergo EcoFast and/or CT scan upon arrival at the General Hospital of Castellón over a period of 3 years (2019-2022). A descriptive analysis of the variables is being performed, using a chi-square test with a statistical significance level of p-value < 0.05. Additionally, the sensitivity, specificity, PPV, and NPV of EcoFast are being calculated.
RESULTS: Out of the 39 patients, 30 are stable, and 32 undergo an EcoFast, with a negative result in 29 individuals, while it is not performed in 7 cases. Among the patients, 31 undergo a CT scan, revealing pathological findings in 25 cases, inconclusive results in 2, and not performed in 9 patients. It is concluded that there is no dependence between the result of the EcoFast and whether a CT scan is performed. The sensitivity is 10%, specificity is 100%, PPV is 100%, and NPV is 10%.
CONCLUSIONS: EcoFast is a test with poor sensitivity, high specificity, high PPV, and low NPV. Therefore, a negative result in the test does not rule out injuries, and it should be confirmed with another test, such as a TC scan, especially in stable patients. [-]
