Efectos de los cuidados paliativos en el domicilio durante la fase agónica

Abstract

Introducción: A medida que aumentan la calidad y la esperanza de vida, las enfermedades se cronifican y aumenta el número de personas que necesitan cuidados paliativos al final de su vida. Estos pacientes necesitan una atención integral y personalizada que aplicada en el domicilio puede facilitarles pasar sus últimos momentos de la forma más confortable y rodeados de los suyos. La fase agónica en concreto engloba hasta los últimos 3 días de vida y puede resultar un proceso muy desagradable a causa de los síntomas que la caracterizan. Metodología: Se ha realizado una revisión integradora de la bibliografía mediante la búsqueda en las bases de datos PubMed, The Cochrane Library, LILACS, Dialnet y Scopus; utilizando tanto lenguaje natural como lenguaje controlado y se han aplicado los filtros automáticos: “free full text”, “in the last 5 years”, “English”, “Spanish” y “humans”. Mediante la lectura de los títulos, resúmenes y finalmente la lectura crítica se realizó la selección de los artículos. Resultados: De los 3106 artículos obtenidos de la búsqueda inicial se seleccionaron 13 finalmente, que se centraban en los cuidados paliativos en el final de la vida. Conclusión: A pesar del deseo de propiciar una muerte en el domicilio que resulte menos agresiva para los pacientes, en ocasiones es imposible facilitar este resultado ya que existen diversos factores que dificultan la muerte en el domicilio. El principal factor influyente es el tipo de enfermedad y su evolución.

Introduction: As the standards of quality of life and life expectancy arise, more illnesses are becoming chronic and the number of people who need palliative care at the end of life keeps growing. These people need an integrated and personalized care which can be applied at home in order to ease the last moments of life as comfortably as possible, surrounded by their loved ones. The agony phase concretely includes up to the last 3 days of life and can become a very disgusting process due to its characteristic symptoms. Methodology: An integrative review has been made by the search in the databases PubMed, The Cochrane Library, LILACS, Dialnet and Scopus; using both controlled and natural language and the following automatic filters have been applied: “free full text”, “in the last 5 years”, “English”, “Spanish” and “humans”. Through the reading of titles, abstracts and finally the critical reading the selection of the articles was made. Outcomes: From the 3106 articles that were originally obtained, 13 were finally selected. These articles were centered around palliative care at the end of life. Conclusion: Even though the health personnel wish to favor a less aggressive death at home for these patients, in some occasions it is impossible to ease these results because there are various factors that influence that hinder the death at home. The main influential factor is the kind of illness that they suffer and its unpredictable evolution.
