Efectos maternofetales de la colestasis en la gestación. Revisión integradora de la literatura

Abstract

Introducción: La colestasis es una patología digestiva con diferentes presentaciones, entre ellas, la Colestasis Intrahepática del Embarazo (CIDE), siendo esta una de las patologías más comunes y graves en las mujeres gestantes. Sus síntomas son múltiples y un diagnóstico precoz de la enfermedad es imprescindible. Objetivo: Realizar una revisión integradora de la literatura sobre los efectos maternofetales que causa la colestasis en el periodo de gestación. Metodología: La búsqueda bibliográfica de los artículos se realizó en diferentes bases de datos, Pubmed, Cochrane, Biblioteca Virtual de Salud (BVS), Cinahl, Scielo y Web of Sciencie (WOS). Se utilizaron los DeCS y se aplicaron los filtros “Free full text”,“ingles” y “español” y por últimos un límite de “5 years”. Resultados: Se obtuvieron 1671 artículos, pasando por diferentes filtros y criterios de exclusión, finalmente fueron 15 artículos los que se emplearon en la revisión y fueron discutidos. La base de datos con el mayor número de artículos fue Pubmed con (n=5) y las bases con menos artículos obtenidos fueron Cochrane y Cinahl con (n=1) Conclusión: Las concentraciones de los ácidos biliares se determinan como la causa principal de la colestasis y de los síntomas de esta. La técnica de diagnóstico precoz más fiable es el control analítico, y en cuanto al tratamiento, el más eficaz del que existen evidencias es el UDCA, sirviendo este para paliar los síntomas y evitar las consecuencias más graves.

Introduction: Cholestasis is a digestive pathology with different presentations, including Intrahepatic Cholestasis of Pregnancy (CIDE), this being one of the most common and serious pathologies in pregnant women. Its symptoms are multiple and an early diagnosis of this is essential. Objective: To carry out an integrative review of the literature on the maternal-fetal effects caused by cholestasis in the gestation period. Methodology: The bibliographic search of the articles is carried out in different databases, Pubmed, Cochrane, Virtual Health Library (VHL), Cinahl, Scielo and Web of Science (WOS). DeCS is used and the filters "Free full text", "English" and "Spanish" are applied and finally a limit of "5 years". Outcomes: 1671 articles were obtained, going through different filters and exclusion criteria, finally there were 15 articles that were used in the review and were discussed. The database with the highest number of articles was Pubmed with (n=5) and the databases with the fewest articles obtained were Cochrane and Cinahl with (n=1) Conclusión: Concentrations of bile acids are determined as the main cause of cholestasis and its symptoms. The most reliable early diagnosis technique is analytical control, and in terms of treatment, the most effective of which there is evidence is udca, serving to alleviate symptoms and avoid the most serious consequences.
