dc.description.abstract | Introducción: En las últimas tres décadas la incidencia del carcinoma escamoso cutáneo (CEC) ha aumentado entre el 50-300%. Además de la exposición solar, estudios previos han sugerido que el consumo de Hidroclorotiazida (HCTZ) se asocia a un aumento de riesgo de CEC. Objetivos: Evaluar la posible asociación entre la toma de HCTZ y el desarrollo de segundos CEC en pacientes con antecedentes de un primer CEC. De esta manera se busca responder a la pregunta de si la retirada de HCTZ puede ser útil para evitar la aparición de nuevos CEC en pacientes que ya han tenido un CEC. Material y métodos: Se seleccionaron todos los pacientes diagnosticados de CEC en 2015 confirmados con estudio histopatológico. Se incluyeron las siguientes variables: edad, sexo, toma o no de HCTZ y desde cuándo en caso afirmativo, características histológicas del primer CEC, aparición de posteriores cáncer de piel, posibles prescripciones de fármacos inmunosupresores y cuáles, posibles trasplantes de órganos y cuáles, meses totales de seguimiento, dosis total acumulada de HCTZ, dosis acumulada hasta aparición de todos y cada uno de los cáncer de piel. Se comparó el grupo que tomando HCTZ desarrolló posteriores CEC con aquel que no desarrolló. Resultados: Se incluyeron 89 pacientes, de los cuales 39 (43’ 8 %) fueron tratados en algún momento con HCTZ y de estos 16 desarrollaron posteriores CEC. En total fueron 65 CEC desarrollados. La media de meses de seguimiento fue de 55 meses (0-84 meses). Conclusiones: No se encuentran diferencias significativas entre la toma de HCTZ y el desarrollo de posteriores CEC, aparición de un CEC de alto riesgo, la aparición de carcinoma basocelular (CBC), la aparición de CEC pobremente diferenciados, aparición de CEC in situ en pacientes con CEC. | ca_CA |
dc.description.abstract | Introduction: In the last three decades, the incidence of squamous cell carcinoma (SCC) has increased between 50-300%. In addition to sun exposure, previous studies have suggested that the consumption of Hydrochlorothiazide (HCTZ) may be associated with an increased risk of SCC. Objectives: To evaluate the possible association between taking HCTZ and the development of second SCC in patients with a history of a first SCC. In this way, we seek to answer the question of whether the withdrawal of HCTZ can be useful to prevent the appearance of new SCC in patients who already had an SCC. Methods: All patients diagnosed with SCC in 2015 confirmed by histopathological studies were selected. The following variables were included: age, sex, taking or not taking HCTZ and since when if so, histological characteristics of the first SCC, appearance of subsequent skin cancer, possible prescriptions of immunosuppressive drugs, possible organ transplants, total months of follow-up, total accumulated dose of HCTZ, accumulated dose until the appearance of each skin cancer. The group that took HCTZ and developed further SCC was compared to the group that did not develop any further SCC. Results: 89 patients were included, of whom 39 (43.8%) were treated with HCTZ at some point. Of these 39 patients, 16 developed subsequent SCCs. 65 SCC were developed in total. The mean number of months of follow-up was 55 months (0-84 months). Conclusions: No significant differences were found between taking HCTZ and the development of subsequent SCC, the appearance of a high-risk SCC, with the appearance of basal cell carcinoma (BCC), the appearance of poorly differentiated SCC, and the appearance of SCC in situ in patients with SCC. Although not significant, there are clear trends in our results. The dose of HCTZ seems to be associated with the development of subsequent SCC and the dose of HCTZ that represents a risk for the development of SCC seems to be considerably lower than that established in previous studies. | ca_CA |
