dc.description.abstract | FUNDAMENTO Y OBJETIVO: La contaminación atmosférica se ha identificado como un factor de riesgo de enfermedad cardiovascular en exposiciones tanto a corto como a largo plazo. El objetivo principal de la presente investigación, será estudiar la posible relación existente entre la exposición a la contaminación atmosférica y la prevalencia de Hipertensión arterial (HTA) e Infarto agudo de miocardio (IAM) en España. MATERIAL Y MÉTODOS: estudio ecológico de la relación entre diferentes contaminantes atmosféricos y la prevalencia de HTA e IAM teniendo en cuenta diversos factores socioeconómicos y antropométricos extraídos de bases de datos disponibles en el Instituto Nacional de Estadística, Ministerio de Transición Ecológica y Desarrollo Rural y Ministerio de Sanidad. RESULTADOS: se ha observado una disminución de la prevalencia de HTA (p<0.05) con cada incremento de 1 µg/m3 de benceno (β=-1.743%, IC del 95%: -2.594, -0.8918), de 1 mg/m3 ug/m3 de CO (β=-7.075, IC del 95%: - 12.325, -1.8253), de 10 µg/m3 de NO (β=-4.412, IC del 95%: -5.975, -2.849 ), 10 µg/m3 de NO2 -3.023 (IC del 95%: -3.975, -2.071), de 20 µg/m3 de NOx (β=- 2.434, IC del 95%: -3.252, -1.617 ) y de 10 µg/m3 de PM10 (β=-2.776, IC del 95%: -3.941, -1.612). Estos resultados son contrarios a la hipótesis de partida y a lo observado en la bibliografía. En el caso del O3 y PM2.5, incrementos de 10 y 5 µg/m3 están asociados a incrementos de prevalencia de HTA (β=1.157%, IC del 95%: 0.251, 2.062) y (β=4.032, IC del 95%: 2.181, 5.884), respectivamente. Respecto a la prevalencia de IAM, se ha observado que cada incremento de 1 µg/m3 de benceno (β =-0.0979%,IC del 95%: -0.1881, -0.00778), de 10 µg/m3 de O3 (β =-0.1284,IC del 95%: -0.2214, -0.03546) y de 5 µg/m3 de PM2.5 (β =-0.19434,IC del 95%: -0.3868, -0.00188) están asociadas significativamente con la prevalencia de IAM (p-value <0.05), de tal manera que a mayor concentración de estos contaminantes disminuye la prevalencia. Estos resultados son contrarios a la hipótesis de partida y a lo observado en la bibliografía. Por otro lado, incrementos de 10 µg/m3 de NO (β = 0.1696,IC del 95%: 0.0106, 0.32853) y de 10 µg/m3 de NO2 (β = 0.1284,IC del 95%: 0.0314, 0.22551) se han asociado con una mayor prevalencia de IAM (p-value <0.05). Se evaluó la estabilidad matemática de los modelos y se observó que no se cumplían todos los requisitos necesarios para dar fiabilidad a los resultados obtenidos, lo cual se resalta como limitación del presente estudio. CONCLUSIONES: el análisis multiparamétrico realizado con las bases de datos públicamente disponibles no ha permitido evaluar si existe una asociación entre la exposición a contaminación atmosférica y la prevalencia del IAM y la HTA. Se recomienda utilizar bases de datos con mayor resolución geográfica para abordar el estudio de los efectos de la contaminación atmosférica en salud cardiovascular. | ca_CA |
dc.description.abstract | BACKGROUND AND OBJECTIVE: Air pollution is the leading cause of early death and preventable disability in today's world. It has been identified as a risk factor for cardiovascular disease in both short- and long-term exposures. The main objective of this research is to study the association between exposure to air pollution and the prevalence of Arterial Hypertension (HT) and Acute Myocardial Infarction (AMI) in Spain. MATERIAL AND METHODS: ecological study of the relationship between different air pollutants and the prevalence of HT and AMI. Data was extracted from publicly available datasets at different Ministries. RESULTS: a decrease in the prevalence of hypertension (p<0.05) has been observed with each increase of 1 μg/m3 of benzene (β=-1.743%, 95% CI - 2.594, -0.8918), of 1 mg/m3 ug/m3 of CO (β=-7.075, 95% CI -12.325, -1.8253), of 10 μg/m3 of NO (β=-4.412, 95% CI -5.975, -2.849 ), 10 μg/m3 of NO2 -3.023 (95% CI -3.975, -2.071), 20 μg/m3 OF NOx (β=-2.434, 95% CI -3.252, -1.617) and 10 μg/m3 of PM10 (β=-2.776, 95% CI -3.941, -1.612) . Regarding the prevalence of AMI, it has been observed that each increase of 1 μg/m3 of benzene (β =-0.0979%,95% CI -0.1881, -0.00778), of 10 μg/m3 of O3 (β =-0.1284,95% CI -0.2214, -0.03546) and of 5 μg/m3 of PM2.5 (β =- 0.19434,95% CI -0.3868, -0.00188) are significantly associated(p-value <0.05) with the prevalence of AMI. The higher the concentration of these pollutants the lower the prevalence. These results are contrary to the starting hypothesis and to what has been observed in the literature. On the other hand, increases of 10 μg/m3 of NO (β = 0.1696,95% CI 0.0106, 0.32853) and 10 μg/m3 of NO2 (β = 0.1284,95% CI 0.0314, 0.22551) have been associated with a higher prevalence of AMI (p-value <0.05). The mathematical stability of the models was evaluated and it was observed that not all the necessary requirements were met to give reliability to the results obtained, which is highlighted as a limitation of the present study. CONCLUSIONS: the multiparametric analysis carried out with publicly available databases has not allowed to evaluate whether there is an association between exposure to air pollution and the prevalence of AMI and hypertension. It is recommended to use databases with higher geographic resolution to address the study of the effects of air pollution on cardiovascular health. | ca_CA |
