Introducción. La insuficiencia cardiaca (IC) es considerada actualmente un problema de
salud pública. Es la primera causa de hospitalización en personas mayores de 65 años y
presenta cifras de supervivencia inferiores al 50% a los 4 años. Objetivos. Describir las
características clínicas y los factores predictores de mortalidad y reingreso de estos
pacientes. Métodos. En este estudio de cohortes históricas se escogieron a los pacientes
que ingresaron por IC en el Hospital Universitario de La Plana del 01/10/2019 hasta el
31/12/2019, y fueron seguidos 12 meses. Resultados: La cohorte final fue de 99 pacientes
con una edad media de 81,48±10,54 años. Fueron mayoritariamente mujeres (65,7%),
con una clasificación de la NYHA III (52,5%). Los predictores independientes de
mortalidad global más importantes fueron: la clasificación de la NYHA (HR=3,405), el
índice de Barthel (HR=0,980), la estancia media en días (HR=0,913), presentar
antecedentes de fibrilación auricular o flutter (HR=10,926) y anemia (HR=3,237).
También haber presentado una saturación de oxígeno disminuida (HR=0,877) y un sodio
bajo (Exp(B)=0,765) en el ingreso; y no haber sido tratado con betabloqueantes
(HR=0,319). Por otro lado, los predictores independientes de reingreso al año fueron la
edad (Exp(B)=0,909), los antecedentes de hipertensión (Exp(B)=26,951) y enfermedad
renal crónica (Exp(B)=7,046), y el tratamiento con furosemida (Exp(B)=9,791). [-]

Introduction. Heart failure (HF) is a public health problem. It is the leading cause of
hospitalization for people over 65 and their 4-years relative survival is less than 50%.
Objectives. To describe the clinical characteristics and to identify predictors of mortality
and readmission of these patients. Methods. This historical cohort study assessed HF
patients from Hospital Universitario de La Plana, admitted between 10/01/2019 and
12/31/2019, that were monitored for 12 months. Results. The final cohort included 99
patients aged 81.48±10.54 years. They were mostly women (65.7%) and 52.5% could be
classified as NYHA class III. The most important independent predictors of overall
mortality weres NYHA classification (HR=3.405), Barthel index (HR=0.980), average
length of stay (HR=0.913), history of atrial fibrillation or flutter (HR=10.926) and anemia
(HR=3.237). Other predictors were decreased oxygen saturation (HR=0.877) and low
sodium (Exp(B)=0.765) on admission; and not taking beta-blockers (HR=0.319). On the
other hand, the independent predictors of readmission at one year were age
(Exp(B)=0.909), history of hypertension (Exp(B)=26.951) and chronic kidney disease
(Exp(B)=7.046), and furosemide treatment (Exp(B)=9.791). [-]
