Según la Organización Mundial de la salud (OMS) la leucemia mieloide aguda tiene que tener una infiltración ≥ 20% de blastos mieloides, o que se hallen alteraciones citogenéticas o moleculares específicas. La LMA es más frecuente cuanto mayor es la edad del paciente, siendo esta un factor pronóstico junto con otros factores del paciente (comorbilidades, estado funcional, etc), factores de la enfermedad, como las alteraciones genéticas moleculares y factores evolutivos. El presente trabajo analiza un total de 134 (38,4%) pacientes < 65 años diagnostivados de LMA entre 1994-2020 con el objetivo de describir las caracterísitcas clínicas, diagnósticas y terapéuticas-evolutivas de este grupo de pacientes. Para seguir una clasificación homogénea se utilizó la European LeukemiaNet (ELN) de 2017 donde se establecen grupos de acuerdo a las alteraciones genéticas y moleculares de cada paciente. La QT intensiva fue el tratamiento de primera línea para los pacientes < 65 años junto con TPH en algunos casos seleccionados con resultados de respuesta completa, recaída, supervivencia y muerte muy similares a lo publicado en la literatura. La edad y las alteraciones genético-molecular son dos de los factores más importantes para determinar la intensidad del tratamiento y el pronósitco.
Igualmente, otras variables que influyen positivamente en la supervivencia son el ECOG <2 (p=0.011), efectuar TPH (p<0.0001) y período de estudio (p<0.0001). Una buena caracterización biológica genética molecular va a mejorar la individualización y resultados terapéuticos. [-]

According to the world Health Organization (WHO), acute myeloid leukaemia must have a ≥ 20% infiltration of myeloblasts, or that cytogenic or specific molecular alterations occur. AML is more frequent the greater the age of the patient is, this being a prognostic factor along with other factors of the patient (comorbidities, functional state, etc.), disease factors, such as genetic and molecular alterations and evolutionary factors. Present work analyses a total of 134 (38.4%) < 65-year-old patients who were diagnosed with AML between 1994 and 2020 in order to describe the clinical, diagnostic and therapeutic and evolutionary features of this group of patients. In order to follow a consistent classification, the European LeukaemiaNet (ELN) of 2017 was used. This establishes groups according to the genetic and molecular alterations of each patient. Intensive chemotherapy was the fist-line treatment for < 65 year-old patients, along with HSCT in some chosen cases. The results of complete response, relapse, survival and death were very similar to those published in the literature.
Age and gentic and molecular alterations are two of the most important factors to determine the intensity and the prognosis of the treatment. Furthermore, other variables that have a positive influence on survival are ECOG <2 (p=0.011), doing HSCT (P<0.0001) and the period of study (p <0.0001) and the period of study (P<0.0001). A good biological genetic and molecular characterisation will improve individualisation and therapeutic results. [-]
