Propuesta de una consulta de telemedicina en cardiología

Abstract

La pandemia provocada por la COVID-19, ha representado un cambio en el paradigma de la atención médica. Generalmente, no se ha podido garantizar una atención óptima a pacientes cardiópatas, retrasando su diagnóstico y/o seguimiento, generando un aumento de la morbilidad y mortalidad, siendo especialmente los enfermos de insuficiencia cardíaca uno de los grupos más perjudicados. Igualmente, la evolución hacia una sociedad más envejecida, sumado al limitado financiamiento del Sistema Nacional de Salud, ha propiciado la primigenia implementación de servicios de telemedicina y manejo telefónico de la cartera de pacientes. Considerando el contexto actual, el siguiente proyecto pretende estandarizar el manejo de pacientes con insuficiencia cardíaca de manera remota. Asimismo, aspira reducir la exposición de tanto profesionales sanitarios como aquellos pacientes más vulnerables, facilitando que los hospitales se centren únicamente en proveer atención médica a aquellos pacientes con patología urgente. Por ello, el siguiente protocolo asistencial pretende orientar la dirección de teleconsultas en Insuficiencia Cardíaca durante las actuales vicisitudes. Análogamente, la sólida búsqueda bibliográfica sistemática realizada avala este protocolo de actuación. Por consiguiente, pretendemos contribuir a la priorización de la asistencia telemática en el Hospital La Plana.

The pandemic caused by COVID-19 has represented a change in the paradigm of medical care. Generally, it has not been possible to guarantee optimal care for patients with heart disease, delaying their diagnosis and/or follow-up, generating an increase in morbidity and mortality, with heart failure patients in particular being one of the most affected groups. Likewise, the evolution towards an ageing society, coupled with the limited funding of the National Health System, has led to the early implementation of telemedicine services and telephone management of the patient portfolio. Considering the current context, the following project aims to standardize the management of heart failure patients remotely. It also aims to reduce the exposure of both healthcare professionals and vulnerable patients, enabling hospitals to focus solely on providing medical care to patients with urgent pathology. Therefore, the following care protocol aims to guide the management of teleconsultations in Heart Failure during the current vicissitudes. Similarly, the solid systematic literature research performed supports this protocol. Therefore, we intend to contribute to the prioritization of telematic assistance in La Plana Hospital.
