dc.description.abstract | RESUMEN Introducción: El tromboembolismo pulmonar agudo (TEP) constituye la tercera causa de mortalidad cardiovascular. La trombolisis sistémica (TLS) es el tratamiento de elección hoy en día en el TEP de alto riesgo, asociándose a una reducción significativa de la presión pulmonar, pero con un aumento de los eventos hemorrágicos. El tratamiento percutáneo del TEP se considera actualmente una alternativa en pacientes con alto riesgo hemorrágico o contraindicación absoluta para la TLS en pacientes de riesgo intermedio-alto y alto. El objetivo del presente estudio es determinar la seguridad y la eficacia del intervencionismo percutáneo (IP) sobre pacientes con TEP de riesgo intermedio-alto. Métodos: Se incluyeron todos los pacientes con TEP de riesgo intermedio-alto que fueron sometidos a IP en el Hospital General Universitario de Valencia y en el Hospital Clínico Universitario de Valencia. Se recogieron variables clínicas, ecocardiográficas, hemodinámicas y bioquímicas pre y post procedimiento, así como eventos adversos durante el seguimiento. Resultados: Desde febrero de 2018 hasta febrero de 2021, 23 pacientes con TEP de riesgo intermedio-alto fueron sometidos a IP. La edad media fue de 64 ± 13 años y el 61% fueron varones. Se observó una mejoría precoz de los parámetros hemodinámicos tras el IP, con una reducción significativa en la mediana de presión de la arteria pulmonar: 30 mmHg (RIQ 30-45) vs. 20 mmHg (RIQ 18-32); p<0,001, pre y post IP respectivamente. Se determinó un descenso significativo de la mediana de NT-proBNP al ingreso y a las 48 horas tras IP, 154 pg/ml (RIQ 70- 250) vs. 42 pg/ml (RIQ 22-87); p=0,032; así como una mejoría de los parámetros ecocardiográficos de ventrículo derecho (VD) a las 72 horas tras el IP: TAPSE 15 mm (RIQ 13-16) vs. 18 mm (RIQ 17-21); p<0,007; y diámetro basal de VD, 50 mm (RIQ 45-52) vs. 40 mm (RIQ 44- 46); p < 0,001, pre y post IP respectivamente. Durante una mediana de seguimiento de 13 meses (RIQ 8-22) no se notificó ningún evento hemorrágico mayor. Conclusiones: El IP en pacientes con TEP de riesgo intermedio-alto es un procedimiento seguro y eficaz, permitiendo una mejoría de los parámetros bioquímicos, hemodinámicos y ecocardiográficos del VD de forma precoz. | ca_CA |
dc.description.abstract | Introduction: Acute pulmonary embolism (PE) is actually the third most frequent cause of cardiovascular mortality. Systemic thrombolysis is nowadays the treatment of choice in high-risk PE, being associated with a significant reduction in pulmonary pressure but also with a significant increase in bleeding events. Percutaneous treatment of PE is currently considered an alternative in patients at high risk of bleeding or an absolute contraindication for systemic thrombolysis in high-intermediate and high-risk patients. The objective of the present study is to determine the safety and efficacy of percutaneous intervention (PI) in patients with intermediate-high risk PE. Methods: We included all patients with high-intermediate-risk PE who underwent percutaneous intervention in the Hospital General Universitario de Valencia and in the Hospital Clínico Universitario de Valencia. Clinical, echocardiographic, hemodynamic, and biochemical variables were collected before and after the procedure, as well as adverse events during follow-up. Results: From February 2018 to February 2021, 23 patients with intermediate-high risk PE underwent PI. The mean age was 64 ± 13 years and 61% were male. An early improvement in hemodynamic parameters was observed after PI, with a significant reduction in the median pulmonary artery pressure: 30 mmHg (IQR 30-45) vs. 20 mmHg (IQR 18-32); p <0.001, pre and post IP respectively. A significant decrease in the median NT-proBNP was determined at admission and 48 hours after PI, 154 pg / ml (IQR 70-250) vs. 42 pg / ml (IQR 22-87); p = 0.032; as well as an improvement in right ventricular (RV) echocardiographic parameters at 72 hours after PI: TAPSE 15 mm (IQR 13-16) vs. 18 mm (IQR 17-21); p <0.007; and basal RV diameter, 50 mm (IQR 45-52) vs. 40 mm (IQR 44-46); p <0.001, pre and post PI respectively. During a median follow-up of 13 months (IQR 8-22), no major bleeding events were reported. Conclusions: PI in patients with intermediate-high risk PE is a safe and effective procedure, allowing an early improvement of the biochemical, hemodynamic and echocardiographic parameters of the RV. | ca_CA |
