Protocolo asistencial para el manjo de las pacientes con cáncer de endometrio

Abstract

INTRODUCCIÓN. El cáncer de endometrio es la neoplasia ginecológica más frecuente en los países desarrollados, fuertemente asociada a la obesidad y edad avanzada. Se diagnostica principalmente en mujeres mayores de 60 años, siendo el síntoma de presentación un sangrado uterino anormal, ante el cual siempre habrá que descartar la presencia de esta neoplasia. OBJETIVOS. Realización de un protocolo asistencial que recoja la estadificación y las diferentes indicaciones terapéuticas del carcinoma de endometrio para su uso en la práctica clínica diaria del área de salud de Castellón. PROCESO DE ELABORACIÓN. Durante 2020-2021 se llevó a cabo una búsqueda sistemática de la mejor evidencia disponible en las distintas fuentes de información y bases de datos, principalmente en PubMed, Cochrane, Sociedades (SEGO, NCCN, FIGO, ESMO-ESGO-ESTRO). Se elaboraron tablas resumen comparativas, seleccionándose las indicaciones de mayor evidencia y se redactó el siguiente protocolo. CONCLUSIÓN. Ante el diagnóstico histopatológico de un carcinoma de endometrio, se realiza un estudio de extensión tumoral y una aproximación por imagen del estadiaje. Estos datos nos permiten clasificar a las pacientes en diferentes grupos de riesgo, que determinarán el abordaje terapéutico y necesidad de tratamiento adyuvante, siendo el tratamiento quirúrgico de elección: histerectomía total con salpingooforectomía bilateral.

INTRODUCTION. Endometrial cancer is the most frequent gynecological neoplasm in developed countries, strongly associated with obesity and advanced age. It is mainly diagnosed in women over 60 years, and the initial symptom is abnormal uterine bleeding, which must always be studied in order to rule out the presence of this neoplasm. OBJECTIVES. Creation of a healthcare protocol that includes the staging and the different therapeutic indications for endometrial carcinoma for use in daily clinical practice in the Castellón health area. ELABORATION PROCESS. During 2020-2021, a systematic search of the best available evidence was carried out in different information sources and databases, mainly PubMed, Cochrane, Societies (SEGO, NCCN, FIGO, ESMO-ESGO-ESTRO). Comparative summary tables were drawn up, selecting the indications with the best evidence and the following protocol was drafted. CONCLUSIONS. When histopathological diagnosis of endometrial carcinoma is made, a tumor extension study must be carried out and an approximation of the staging is made by imaging. This data allows us to classify patients into different risk groups, which will determine the therapeutic approach and the need for adjuvant treatment. The treatment of choice is the surgical approach with total hysterectomy and bilateral salpingo-oophorectomy.
