dc.description.abstract | Introducción: En la era de la medicina personalizada, las guías clínicas siguen basando su plan terapéutico escalonado en el control del asma, no siendo eficaz en un porcentaje importante de pacientes. Ante esta situación, es necesario integrar en la práctica clínica diaria biomarcadores capaces de identificar fenotipos y predecir el curso de la enfermedad y la respuesta a una terapia determinada. Objetivos: La finalidad de esta revisión es evaluar la aplicabilidad de los biomarcadores definidos en el asma. Metodología: Se revisaron 7 ensayos clínicos y 15 estudios transversales mediante una búsqueda bibliográfica en tres bases de datos: Pubmed, Biblioteca Cochrane y ClinicalTrials.gov. Se elaboraron unas tablas que recogían la información más relevante de cada estudio y otra tabla comparativa de las distintas utilidades de los biomarcadores. Resultados: El FeNO junto con los eosinófilos en sangre y la periostina son el mejor indicador para predecir el riesgo de exacerbaciones del asma. La periostina es un buen biomarcador para identificar el deterioro de la función pulmonar. Los eosinófilos en sangre son un biomarcador válido para definir el control de la enfermedad. Conclusión: Los biomarcadores del asma podrían acelerar el camino a una medicina de precisión y de manejo clínico, aunque se precisa un mayor número de estudios para definir umbrales diagnósticos de los distintos biomarcadores. | ca_CA |
dc.description.abstract | Introduction: In the era of personalized medicine, clinical guidelines continue to base their stepped therapeutic plan on asthma control, not being effective in a significant percentage of patients. Given this situation, it is necessary to integrate biomarkers capable of identifying phenotypes and predicting the course of the disease and the response to a determined therapy into daily clinical practice. Objectives: The purpose of this review is to evaluate the applicability of the biomarkers of asthma. Methodology: 7 clinical trials and 15 cross-sectional studies were reviewed through a bibliographic search in three databases: Pubmed, Cochrane Library and ClinicalTrials.gov. We created tables that collected the most relevant information from each study and another table that compare the different utilities of biomarkers. Results: FeNO with blood eosinophils and periostin are the best indicator to predict the risk of asthma exacerbations. Periostin is a good biomarker to identify impaired lung function. Blood eosinophils are a valid biomarker to define asthma control. Conclusion: Asthma biomarkers could accelerate the path to precision medicine and clinical management, although a greater number of studies are required to define diagnostic thresholds for the different biomarkers. | ca_CA |
