Introducción: la enfermedad ósea de Paget (EOP) es un trastorno metabólico que se
caracteriza por una estructura ósea desorganizada que predispone a deformidades y fracturas.
Los estudios sugieren que en los últimos años la EOP ha mostrado una tendencia a presentarse
de forma más tardía y con una menor prevalencia y severidad.
El objetivo de este estudio fue describir las diferencias en cuanto a severidad y edad al
diagnóstico entre las tres últimas décadas. Paralelamente, se analizó la influencia del género
en la severidad y cambios seculares.
Material y métodos: Se realizó un estudio observacional descriptivo y retrospectivo, en el que
se incluyó a todos los pacientes diagnosticados de EOP en el Hospital General Universitario de
Castellón entre febrero de 1990 y febrero de 2020. Los niveles séricos de fosfatasa alcalina
total (FAT), el número de localizaciones y complicaciones y la distribución gammagráfica se
estudiaron como variables de severidad.
Resultados: Se incluyeron 61 casos de EOP, 28 mujeres y 33 hombres. La edad media al
diagnóstico se ha visto incrementada de forma significativa, siendo de 48,1 años en el primer
periodo, de 63,4 años en el segundo y de 69,7 años en el último (p<0,02). El descenso de la
severidad se define por la disminución significativa de la media de 3 huesos afectos en el
segundo decenio a 1,72 en el último (p<0,02), junto con una tendencia positiva a la
distribución monostótica (p<0,013). Ambos géneros mostraron diferencias en la distribución
de las variables de extensión y actividad de la enfermedad, aunque sin llegar a alcanzar
significación estadística (p>0,05).
Conclusiones: los resultados sugieren una tendencia secular de la EOP a presentarse en edades
más tardías con una menor extensión gammagráfica y, por tanto, con una menor severidad de
la enfermedad al diagnóstico. [-]

Introduction: Paget’s disease of bone (PDB) is a benign metabolic disorder characterized by
disorganization of the bone structure with a consequent predisposition to deformities and
fractures. Studies suggest that in recent years PDB has tended to present itself later and with
lower prevalence and severity.
The goal of this study was to describe the differences in severity and age at the time of
diagnosis experienced by patients with PDB during the last three decades. In parallel, the
influence of gender on severity and secular changes was analyzed.
Methods: An observational, descriptive, retrospective study was performed. It includes all
cases of PDB diagnosed from February 1990 until February 2020 at the Hospital General
Universitario de Castellón. Gammagraphic distribution, number of locations, number of
complications and plasma total alkaline phosphatase were used as indices of severity.
Results: During this period, 61 cases of PDB were diagnosed, of which 36 belonged to men and
25 to women. The mean age at diagnosis has increased significantly, being 48.1 years in the
first period, 63.4 years in the second and 69.97 years in the last (p<0.02). The decrease in
severity between the last two decades is defined by a significant decrease in the mean of 3
affected bones in the second decade to 1.72 in the last (p<0.02) with a positive trend to
monostotic distribution (p<0.013). Differences were found between both genders in the
distribution of the disease’s extension and activity variables, but without reaching statistical
significance (p>0.05).
Conclusions: the results suggest a secular trend of PDB to present at later ages with less
extensive skeletal involvement and a declining severity of Paget's disease. [-]
