Introducción y objetivos: el síncope no filiado y bloqueo de rama (BR) representa un
reto diagnóstico. Se recomienda un estudio electrofisiológico (EEF) para valorar
implante de marcapasos (MP). El objetivo del estudio fue elaborar una puntuación
predictiva de implante de MP para estratificar el riesgo y refinar la estrategia diagnóstico
terapéutica.
Material y métodos: estudio observacional, unicéntrico y retrospectivo. Se incluyeron
todos los pacientes con síncope y BR remitidos a la Unidad de Arritmias del Hospital
General Universitario de Castellón desde 2004. Se realizó un análisis mediante
regresión logística. Se asignó una puntuación a cada variable predictiva independiente
en función del valor de su Odds Ratio (OR).
Resultados: se reclutaron 239 pacientes. El 71% (n=169) requirió implante de MP. 1
variable clínica y 3 electrocardiográficas se asociaron de manera independiente con el
implante de MP: traumatismo corporal (OR 3,5; p=0,001), PR ≥ 200 ms (OR 4,3
p=0,000), bloqueo completo de rama derecha con hemibloqueo anterior o posterior (OR
4,9; p=0,001) y bloqueo completo de rama izquierda (OR 2,6; p=0,034). Se asignó a
cada variable un valor ajustado a su OR y la puntuación elaborada estratificó a los
pacientes en bajo riesgo (n=61; 0-3,5 puntos), riesgo intermedio (n=52; 4,5-6 puntos) y
alto riesgo (n=126; 7-13 puntos) con porcentajes de implante de MP del 41, 67 y 86,5%
respectivamente.
Conclusión: la puntuación permite estratificar a los pacientes e individualizar la
estrategia -como podría ser el implante directo de MP en pacientes de alto riesgoconsiguiendo reducir el número de EEF. [-]

Background and purpose: patients with unknown syncope and bundle-branch block
(BBB) pose a diagnostic challenge. Therefore, it is recommended to perform an
electrophysiological study (EPS) to decide if a pacemaker (PM) implantation is needed.
The objective of this study was to create a predictive score for pacemaker (PM) implant
which stratifies patients according to their risk and refine the diagnostic-therapeutic
strategy.
Methods: observational, single-institution and retrospective study. It includes all the
patients with syncope and BBB who were sent to the Arrhythmia Unit of the General
University Hospital of Castellón since 2004. A multivariate analysis with a logistic binary
regression was performed. A value was assigned to each independent predictive
variable according to their Odds Ratio (OR).
Results: a total of 239 patients participated in the clinical research. The 71% (n=169)
needed a PM. 1 clinical variable and 3 electrocardiographic variables were associated
independently with PM implant: corporal trauma (OR 3,5; p=0,001), PR ≥ 200 ms (OR
4,3 p=0,000), right bundle-branch block with left anterior or posterior fascicular block (OR
4,9; p=0,001) and left bundle-branch block (OR 2,6; p=0,034). An adjusted value from
the OR was ascribed to each variable and the score stratified patients between low risk
(n=61; 0-3,5 points), intermediate risk (n=52; 4,5-6 points) and high risk (n=126; 7-13
points) with PM implant percentages from 41, 67 and 86,5% respectively.
Conclusion: the score allows us to stratify the patients and personalize the strategy -
like direct PM implant in those with high risk- reducing the number of EPS. [-]
