dc.description.abstract | En los últimos años se han reportado casos de disfunción tubular proximal como efecto secundario del VPA. Éste no es detectado de forma habitual debido a su poca expresividad inicial y podría tener consecuencias en el paciente a largo plazo. Objetivos: El objetivo principal es evaluar signos de DTP en pacientes tratados con VPA. El objetivo secundario es identificar factores de riesgo de desarrollo de la DTP inducida por el VPA. Métodos: Estudio observacional transversal analítico que comparará la afectación tubular proximal en 32 pacientes tratados con VPA y en 24 no tratados. Los datos se obtendrán mediante revisión de la historia clínica y análisis bioquímico de muestra de orina. Resultados: Un 28,1% de casos presentaba proteinuria tubular patológica frente a un 12,5% de controles (OR2,74 IC95% 0,69-10,59) (p0,200), siendo los niveles del cociente beta-microglobulina/cr superiores en casos que en controles (0,41mg/g vs 0,13mg/g) (p0,077). La diferencia de edad estadísticamente significativa entre ambos grupos puede constituir un sesgo de confusión. Los factores de riesgo que se han asociado de forma significativa al diagnóstico de proteinuria tubular en los casos son: encamamiento prolongado (p0,044), alimentación mediante nutrición enteral (p0,044) e ingesta de otros antiepilépticos (en el límite de la signifcación, p0,076). Conclusiones: Los datos de nuestro estudio sugieren que los pacientes en tratamiento con VPA tienen un riesgo incrementado de desarrollar proteinuria tubular, sobretodo si presentan factores de riesgo asociados (encamamiento prolongado y nutrición enteral), siendo en estos últimos recomendable el despistaje de nefropatía tubular. | ca_CA |
dc.description.abstract | In recent years cases of proximal tubular dysfunction have been reported as a secondary effect associated to it. This secondary effect isn’t usually detected by the physician because of its low initial significance and it could have consequences for the patient in the long term. Objectives: The main objective is to evaluate signs of proximal tubule dysfunction in patients treated with VPA. The secondary objective is to identify risk factors which my be involved in the development of DPT caused by VPA. Methods: Cross-sectional observational analytical study where the PTD is compared between 32 patients treated with VPA and 24 patients not treated with VPA. The data will be obtained by reviewing the clinical history of the patient and by the urine’s biochemical analysis. Results: 28.1% of the cases presented pathological tubular proteinuria compared to a 12.5% of controls (OR2.74 95% CI 0.69-10.59) (p0.200), with beta-microglobulin/cr ratio levels being higher in cases than in controls (0.41mg/g vs 0.13mg/g) (p0.077). The statistically significant difference in age between both groups may constitute a confounding bias. The risk factors that have been significantly associated with the diagnosis of tubular proteinuria in cases are prolonged bedriddenness (p0.044), feeding through enteral nutrition (p0.044) and the intake of other antiepileptic drugs at the limit of significance ( p0.076). Conclusions: The data from our study suggests that patients on VPA are at increased risk of developing tubular proteinuria, especially if they have associated risk factors (prolonged bedding and enteral nutrition). In these latter patients tubular nephropathy screening is recommended. | ca_CA |
