dc.description.abstract | La terapia anticoagulante oral (TAO) es un tratamiento crónico relativamente frecuente, especialmente en pacientes añosos. La principal indicación en Europa es la presencia de fibrilación auricular (FA). El envejecimiento progresivo de la población y el incremento de la esperanza de vida se añaden a un aumento de incidencia de la FA con la edad, datos que sugieren que aumentará la prevalencia en el uso de estos fármacos. A grandes rasgos, encontramos 3 grupos anticoagulantes, los antagonistas de la vitamina K (AVK), los anticoagulantes de acción directa (ACOD) y por ultimo las heparinas. Al tratarse de un tratamiento crónico hay pacientes que se someterán a procedimientos invasivos en los que se verá incrementado el riesgo hemorrágico y, por lo tanto, será necesario retirar la anticoagulación. Esta retirada del tratamiento, a su vez incrementa el riesgo tromboembólico siendo necesario un adecuado manejo farmacológico para reducir la aparición de las posibles complicaciones. El presente Trabajo de Fin de Grado ha consistido en la realización de un protocolo para el manejo de la anticoagulación frente a procedimientos invasivos adecuado al Hospital Universitario de la Plana. El objetivo principal que se tiene con el mismo es estandarizar el manejo anticoagulante frente a este tipo de intervenciones consiguiendo disminuir el riesgo hemorrágico y tromboembólico del paciente. Para la ejecución del protocolo se ha llevado a cabo una búsqueda bibliográfica sistemática sobre la evidencia científica publicada hasta la fecha adaptándola al hospital mencionado. Finalmente se han generado una serie de algoritmos terapéuticos para facilitar la aplicación del protocolo junto con una serie de tablas de riesgo hemorrágico individualizadas para cada especialidad. | ca_CA |
dc.description.abstract | Oral anticoagulant therapy (OAT) is a relatively frequent chronic treatment, especially in elderly patients. The main indication in Europe is the presence of atrial fibrillation (AF). The progressive aging of the population and the increase in life expectancy can obtain an increase in the incidence of AF with age, data that may increase the prevalence in the use of these drugs. Broadly speaking, we found 3 anticoagulant groups, vitamin K antagonists (AVK), direct-acting anticoagulants (DAAC) and finally heparins. As it is a chronic treatment, there are patients who will undergo invasive procedures in which the bleeding risk will be increased and, therefore, it will be necessary to withdraw the anticoagulation. This withdrawal of treatment, in turn, increases the thromboembolic risk, requiring adequate pharmacological management to reduce the appearance of possible complications. The present Final Degree Project has consisted in the realization of a protocol for the management of anticoagulation against invasive procedures, suitable for La Plana Hospital. Its main objective is to standardize anticoagulant management against this type of intervention, reducing the patient's bleeding and thromboembolic risk. For the execution of the protocol, a systematic bibliographic search was carried out on the scientific evidence published to date, adapting it to the mentioned hospital. Finally, a series of therapeutic algorithms have been generated to facilitate the application of the protocol together with a series of individualized bleeding risk tables for each specialty. | ca_CA |
