dc.description.abstract | Diabetes mellitus (DM) is a condition that affects the carbohydrate, lipid and protein metabolism and is characterized by the presence of elevated plasma glucose levels. Since DM has a diverse etiopathogenesis, we must classify it according to the different types (type 1, type 2, gestational diabetes mellitus, among others). Diabetes mellitus is the most common metabolic condition of pregnancy. In Spain, the estimated prevalence of gestational diabetes (GDM) is around 12%. Most of these patients have a genetic or metabolic predisposition to diabetes and are unable to adequately compensate for the diabethogenic effects of pregnancy. GDM is defined as a glucose intolerance that is first detected during pregnancy, regardless of severity, insulin requirement or postpartum persistence. GDM leads to an increase in maternal and fetal morbility through an increased risk of urinary tract infections, vaginal candidiasis, polyhydramnios, hypertension, prematurity, malformations and/or abortions, restricted intrauterine growth, macrosomia (dystocias, obstetric traumatism and increased rate of caesarean sections), risk of loss of fetal well-being before or during delivery, hypertrophic mycardiopathy and fetal immaturity. For all these reasons, it is necessary to have a diabetes screening for all pregnant women, as well as an adequate management and treatment of the condition. The aim of this present final year project is to develop a clinical practice guide for the Gynaecology and Obstetrics Department of the Hospital Universitario de La Plana (Castellón). The aim of this guide is to standardize the appropriate management of pregnant women suffering from diabetes mellitus. In order to achieve this, a systematic search has been carried out to obtain the current scientific evidence on the subject, in order to be able to apply it to our healthcare system. | ca_CA |
dc.description.abstract | La diabetes mellitus (DM) es una enfermedad que afecta al metabolismo de los hidratos de carbono, lípidos y proteínas, con una etiopatogenia diversa que dará lugar a los distintos tipos en su clasificación (tipo 1, tipo 2, gestacional, otros), y se caracteriza por la presencia de niveles elevados de glucosa plasmática. La DM constituye la alteración metabólica que más frecuentemente se asocia al embarazo. En España, la prevalencia de diabetes gestacional (DG) se estima en torno al 12%, la mayoría de estas pacientes tienen predisposición genética o metabólica a la diabetes, incapaces de compensar de forma adecuada los efectos diabetógenos del embarazo. Se denomina diabetes gestacional a aquella diabetes diagnosticada por primera vez durante la gestación, independientemente de la gravedad, del requerimiento insulínico o de la persistencia posparto. La DG conlleva un incremento de la morbimortalidad materna y fetal, mediante un mayor riesgo de infecciones urinarias, candidiasis vaginal, polihidramnios, estados hipertensivos del embarazo, prematuridad, malformaciones y/o abortos, crecimiento intrauterino restringido, macrosomía (distocias, traumatismo obstétrico y aumento de la tasa de cesáreas), riesgo de pérdida de bienestar fetal ante o intraparto, miocardiopatía hipertrófica e inmadurez fetal. Por todo ello es necesario un cribado de diabetes a todas las gestantes, así como un manejo y tratamiento adecuado de la enfermedad. El presente Trabajo Final de Grado ha consistido en la realización de una guía de práctica clínica para el servicio de Ginecología y Obstetricia del Hospital Universitario de La Plana. El objetivo de esta guía es estandarizar el adecuado diagnóstico y manejo de la embarazada con diabetes mellitus. Con este fin se ha realizado una búsqueda sistemática de la evidencia científica actual sobre el tema, para poder aplicarla a nuestro sistema y departamento de salud. | ca_CA |
