Análisis de la correlación cuantitativa entre resultados de la prueba de la tuberculina y el test quantiferon-tb usadas en estudios de contactos de tuberculosis para el diagnóstico de la infección tuberculosa latente

Abstract

Planteamiento del problema: En el diagnóstico actual de infección tuberculosa latente (ITBL) se están utilizando la PT y el QuantiFERON Gold In Tube (QTF-GIT).Ambas ofrecen resultados cuantitativos. Objetivo: Estimar el grado de correlación la PT el QTF-GIT, según el antecedente de vacunación con BCG. Material y métodos: Estudio transversal, con 625 contactos de enfermos con tuberculosis a los que se les practicó en paralelo las 2 pruebas diagnósticas. Se realizó un análisis gráfico y estadístico de la relación entre PT y QFT-GIT como variables cuantitativas y semi-cuantitativas. Se calcularon los coeficientes de correlación de Spearman y Pearson, representando la recta de regresión y curvas Loess. Resultados: Tanto en el análisis gráfico como en el numérico se ha observado una correlación positiva entre la Pt y el QTF-GIT. La prevalencia de positivos QTF-GIT aumentó en cada categoría de mm de PT. Los coeficientes de correlación entre PT y QFT-GIT fueron 0.598 (Spearman) y 0.637 (Pearson). Los resultados presentaron diferencias según la BCG. Conclusiones: Estas pruebas suelen interpretarse como positiva /negativa, pero se ha observado una correlación positiva entre ambas. Con algunas diferencias entre vacunados y no vacunados. Estos test deberían ser considerados como cuantitativos en la valoración de los resultados

Problem Statement: PT and QuantiFERON Gold In Tube (QTF-GIT) are being used in the current diagnosis of latent tuberculosis infection (LTBI). Both tests provide quantitative results. A graphical and statistical analysis of the relationship between PT and QFT-GIT as quantitative and semi-quantitative variables was performed. The correlation coefficients of Spearman and Pearson were calculated, representing the regression line and Loess curves. Results: A positive correlation between Pt and QTF-GIT was observed both in the graphical and in the numerical analysis. The prevalence of QTF-GIT positive increased in each category of PT mm.
