Backround: asthma is a chronic and heterogeneous inflammation of the airway.
It encompasses the T2 and non-T2 endotypes. T2 is predominant therefore the
physiopathogenic knowledge and therapeutic development of T2 is much greater.
This situation, added to the lack of knowledge of the exact mechanisms that guide
non-T2 inflammation and a poor response to traditional treatments, results in an
asthma generally severe and difficult to treat.
Objective: review studies that evaluate drugs with therapeutic targets typical of
non-T2 severe asthma, whether biological or non-biological. As well as review
the usefulness of these drugs in the treatment of non-T2 endotype.
Methods: 11 clinical trials were reviewed. They were selected according to the
eligibility criteria after a bibliographic search in the PubMed, Cochrane Library
and Scopus databases. Then a data extraction table was made with the main
findings. Finally, a risk of bias analysis and an assessment of the quality of the
evidence were performed.
Results: anti-TNFα, anti-IL-17, anti-IL-23, anti-IL-8, and anti-FLAP biologic
therapies may not be useful in the treatment of non-T2 asthma. On the other
hand, azithromycin, anti-IL1 and anti-TSLP therapies showed their possible
usefulness as treatment. The safety of anti-IL-17/anti-IL-13 drugs cannot be
declared due to the risk of immunogenic consequences.
Conclusion: despite the development of effective drugs there is no established
therapy for low T2 asthma. For this reason it is vital to investigate the
inflammatory pathways of the non-T2 endotype in order to discover new
therapeutic targets and, therefore, more effective pharmacological measures. [-]
