Tratamientos y recomendaciones disponibles para el manejo de la urticaria crónica inducible: una revisión sistemática

Abstract

Introducción: Las urticarias crónicas inducibles son un tipo de urticaria que aparecen, a diferencia de las urticarias espontáneas, tras un desencadenante específico. Han de ser reproducibles mediante pruebas de provocación. Hay diferentes subtipos y cada uno de estos responden a un estímulo diferente. Es una enfermedad que afecta mucho a la calidad de vida de los pacientes y hoy en día no hay un algoritmo terapéutico establecido. Objetivos: Elaborar una revisión sistemática sobre los diferentes tratamientos disponibles para el abordaje de las CIndUs, así como estudiar el nivel de evidencia y grado de recomendación que estos presentan. Métodos: Se realizó una búsqueda bibliográfica en diferentes fuentes de información tras establecer unos criterios de inclusión que cumplieran los objetivos planteados. Posteriormente se evaluó el riesgo de sesgo, el nivel de evidencia y grado de recomendación de los artículos finalmente seleccionados. Resultados: Se incluyeron 12 artículos en la revisión. El riesgo de sesgo varía entre los diferentes estudios. La mayoría de los autores recomiendan la evitación del desencadenante como medida principal para tratar las CIndUs. Los fármacos con los que se obtienen los mejores resultados son los antihistamínicos-H1 de 2ª generación y el Omalizumab. Conclusión: La falta de ECAs provoca que los pacientes sean sometidos a tratamientos que presentan un nivel de evidencia muy baja. Todos los autores coinciden en que se necesitan más estudios para conocer en profundidad los diferentes subtipos de CIndUs y cómo abordarlos.

Introduction: Chronic inducible urticaria is a type of urticaria that appears, unlike spontaneous urticaria, after a specific trigger. They must be reproducible by provocation tests. There are different subtypes, each of which responds to a different stimulus. It is a disease that greatly affects the quality of life of patients and nowadays there is no established therapeutic algorithm. Objectives: To carry out a systematic review of the different treatments available for the treatment of Chronic Inducible Urticaria, as well as to study the level of evidence and degree of recommendation that they present. Methods: A bibliographic search was conducted using different sources of information after establishing inclusion criteria that met the proposed objectives. Subsequently, the risk of bias of the articles finally selected and the level of evidence and degree of recommendation were evaluated. Results: Twelve articles were included in the review. The risk of bias differs among the different studies. Most authors recommend trigger avoidance as the main measure to treat Chronic Inducible Urticaria. The drugs with the best results are 2nd generation H1-antihistamines and Omalizumab. Conclusion: The lack of randomized clinical trials (RCTs) result in patients being subjected to treatments with a very low level of evidence. All authors agree that further studies are needed to understand in depth the different subtypes of Chronic inducible urticaria and how to address them.
