Las fracturas distales de húmero son un grupo de fracturas poco frecuentes en el
paciente adulto, con una distribución bimodal centrada alrededor de los hombres jóvenes
entre 12-19 años y, más marcadamente, la población femenina mayor de 80 años.
Con el objetivo de conocer las consecuencias de este tipo de fractura según su
manejo terapéutico en el Departamento de Salud de Castellón (281000 habitantes), se ha
llevado a cabo un estudio descriptivo en el período comprendido entre los años 2008 y
2019.
Se han incluido todos los pacientes mayores de 14 años con diagnóstico
radiológico confirmado, registrados en SIA-Abucasis / Orion / Centricity Zero Footprint
Universal Viewer. El total asciende a 47 casos. El 72% de los pacientes eran mayores de
64 años y un 64% de ellos eran mujeres.
Se objetivó una pérdida de 3,4 grados de flexión por cada incremento de 10 años
de edad en el momento de la fractura. La diferencia de flexión entre los pacientes
tratados mediante el tratamiento estándar (reducción abierta con fijación interna) frente al
resto fue de +22.4º. Se observó una incidencia de complicaciones 1,8 veces mayor en el
grupo de tratamiento estándar frente al resto, no estadísticamente significativa. La mayor
cumplimentación de las historias clínicas se asoció a una ganancia de 10 grados de
flexión, y 5 de extensión, ambas no significativas estadísticamente.
Las fracturas distales de húmero en adultos siguen planteando un problema de
resolución compleja, con resultados generalmente subóptimos, probablemente en relación
al tipo de paciente en el que se presentan, y la complejidad anatómica de la articulación
afectada. Se interpretan éstos hallazgos, y se proponen futuras líneas de estudio. [-]

Distal humerus fractures are an infrequent group of fractures in the adult patient.
They occur in a bimodal distribution, with a first peak of incidence around young males
aged 12 to 19 years old, and a second, much larger peak in the population over 80 years
old, particularly in females.
The aim of the study was to ascertain the consequences of this type of fracture
depending on the therapeutic approach utilized, in the Department of Health of Castelló
(281000 inhabitants). To this end, a retrospective study spanning from 2008 to 2019 has
been conducted.
All the radiologically diagnosed patients above the age of 14, registered within SIAAbucasis / Orion / Centricity Zero Footprint Universal Viewer have been included in the
study. From a total of 47 cases, 72% were above 64 years of age, and 64% of them were
women.
A loss of 3,4 degrees of flexion per every 10 more years of age at the moment of
the fracture was identified. There was a 22’4º increase of maximum flexion in the patients
treated by the standard method compared to alternative management options. Incidence
of complications was 1,8 times higher in the standard treatment group (statistically non
significative) . A more thorough completion of the patient’s clinical history was associated
with an average gain of 10 degrees of flexion, and 5 of extension (both differences
statistically non significative).
Distal humerus fractures remain a clinical problem of complex solution, with
associated sub par results, due perhaps to the type of patient it is most common in, as well
as the anatomical complexity of the affected joint. The observed findings are here
interpreted, and subsequent lines of studies are proposed. [-]
