dc.description.abstract | Antecedentes: El fracaso renal agudo (FRA) es una complicación frecuente e infradiagnosticada en pacientes ingresados en unidades de cuidados intensivos (UCI). La detección de proteinuria por tira reactiva y el aumento del cociente microalbúmina/creatinina en orina podrían diagnosticar el FRA en estadios precoces. Objetivos: Evaluar si la presencia de proteinuria por tira reactiva y el cociente microalbúmina/creatinina en orina permiten establecer el diagnóstico de FRA antes que los criterios de las guías de la Kidney Disease Improving Global Outcomes (KDIGO) y comparar la incidencia de FRA según ambos procedimientos. Métodos: Estudio observacional, prospectivo, descriptivo y analítico realizado en la UCI del Hospital General Universitario de Castellón (HGUCS) entre diciembre de 2019 y diciembre de 2020. Se analizaron variables demográficas, clínicas, de gravedad y analíticas para el seguimiento de la función renal durante el ingreso. Resultados: El 42,4% de los pacientes presentaron criterios de FRA según las guías KDIGO. Al tercer día, los valores de proteinuria en los pacientes con criterios de FRA fueron superiores con respecto al grupo sin criterios de FRA [79 mg/dl (45-144) vs 43,96 ± 22 mg/dl; p < 0,05]. Un valor de proteinuria superior a 64,5 mg/dl permitió clasificar adecuadamente al 71,4% de los pacientes que presentaban FRA (AUC = 0,766; p = 0,031; IC 95% 0,506-1). El cuarto día, se encontraron valores de proteinuria y el cociente microalbúmina/creatinina en orina mayores en el grupo con criterios de FRA, resultando significativos. Conclusiones: Comparado con la presencia de criterios KDIGO, la detección de proteinuria por tira reactiva y el cociente microalbúmina/creatinina en orina no han permitido establecer el diagnóstico de FRA. Tampoco se han visto diferencias en la incidencia. | ca_CA |
dc.description.abstract | Introduction: Acute kidney injury (AKI) is a frequent and underdiagnosed complication in patients admitted to intensive care units (ICU). Detection of proteinuria by dipstick and an increased urine microalbumin/creatinine ratio could diagnose AKI at early stages. Objectives: To evaluate whether the presence of proteinuria by dipstick and the urine microalbumin/creatinine ratio allow establishing the diagnosis of AKI before the Kidney Disease Improving Global Outcomes (KDIGO) guidelines criteria and to compare the incidence of AKI according to both procedures. Methods: Observational, prospective, descriptive and analytical study conducted in the ICU of the General University Hospital of Castellon between December 2019 and December 2020. Demographic, clinical, analytical variables and severity scores were analysed for the follow-up of renal function during admission. Results: 42,4% of the patients presented AKI criteria according to the KDIGO guidelines. On the third day, proteinuria values in patients with AKI criteria were higher than in the group without AKI criteria [79 mg/dl (45-144) vs 43,96 ± 22 mg/dl; p < 0,05]. A proteinuria value higher than 64,5 mg/dl allowed the proper classifications of 71,4% of patients with AKI (AUC = 0,766; p = 0,031; IC 95% 0,506-1). On the fourth day, proteinuria values and the urine microalbumine/creatinine ratio were higher in the group with AKI criteria thus making these results statistically significant. Conclusions: Compared to the presence of KDIGO criteria, the detection of proteinuria by dipstick and the urine microalbumine/creatinine ratio did not allow the diagnosis of AKI to be established.
