dc.description.abstract | Introducción: La lactancia materna confiere números beneficios al lactante y con frecuencia se ve comprometida por la mastitis, una enfermedad común entre las mujeres que amamantan. El tratamiento tradicional de esta enfermedad consiste en el vaciamiento eficaz de la leche, analgesia y antibióticos. Estos últimos generan varios problemas, entre ellos, los agentes que habitualmente causan mastitis muestran frecuentemente resistencia a la terapia con antibióticos. En este contexto, resulta especialmente atractivo el desarrollo de nuevas estrategias para el manejo de la mastitis, basadas en probióticos. Objetivo: estudio de la evidencia científica disponible acerca del efecto del consumo de probióticos por madres lactantes para el tratamiento y prevención de la mastitis lactacional, así como su efecto sobre la composición inmunológica de la leche. Métodos: Se han incluido 15 artículos que son el resultado de una búsqueda bibliográfica en las bases de datos Pubmed, Scopus, Cochrane, Web of Science y SCIELO utilizando combinaciones de los siguientes términos: “Probiotics”, “Probióticos”, “Mastitis”, “Lactancia”, “Breastfeeding”, “Breastmilk”, “Lactation”. Resultados: La evidencia disponible demuestra una disminución en la concentración bacteriana de la leche materna, una mejoría de los síntomas de mastitis, así como una disminución de IL-8 en las mujeres que reciben probióticos. Se observa un menor consumo de fármacos antiinflamatorios y analgésicos en mujeres que toman probióticos. La administración de probióticos puede modificar la composición de la leche. Se han visto cambios en el perfil lipídico, en los niveles de IgA y en los de otros factores inmunológicos. Los resultados de los estudios son variables, se observa una disminución o aumento de los factores dependiendo del estudio. Conclusiones: Los estudios demuestran resultados prometedores a favor del impacto positivo y bien tolerado del consumo de probióticos pero el rigor, la calidad y el informe de los ensayos se vieron comprometidos y no parecen suficientes para respaldar un cambio en el tratamiento habitual de la mastitis lactacional. La suplementación con probióticos parece influir en los factores inmunológicos de la leche materna, pero se necesita más investigación rigurosa para confirmar la evidencia emergente. | ca_CA |
dc.description.abstract | Introduction: Breastfeeding confers numerous benefits to the infant and is frequently compromised by mastitis, a common disease among breastfeeding women. The traditional treatment of this disease consists of effective emptying of milk, analgesia, and antibiotics. These generate several problems, among them, the agents that usually cause mastitis frequently have resistance to antibiotic therapy. In this context, the development of new strategies for the management of mastitis, based on probiotics, is especially attractive. Objective: a study of the scientific evidence available about the effect of the consumption of probiotics by lactating mothers for the treatment and prevention of lactational mastitis, as well as its effect on the immunological composition of milk. Methods: 15 articles have been included that are the result of a bibliographic search in the databases Pubmed, Scopus, Cochrane, Web of Science and SCIELO using combinations of the following terms: "Probiotics", "Probiotics", "Mastitis", "Lactation", "Breastfeeding", "Breastmilk", "Lactation". Results: The available evidence shows a decrease in the bacterial concentration of breast milk, an improvement in mastitis symptoms, as well as a decrease in IL-8 in women taking probiotics. Lower consumption of anti-inflammatory and analgesic drugs is observed in women taking probiotics. The administration of probiotics could modify the composition of the milk. Changes have been seen in the lipid profile, in the levels of IgA and other immunological factors. The results of the studies are variable, a decrease or increase of the factors is observed depending on the study. Conclusions: Studies show promising results in favor of the positive and well-tolerated impact of probiotic consumption, but the rigor and quality of the trials were compromised and do not seem sufficient to support a change in the current treatment of lactational mastitis. Probiotic supplementation during pregnancy and lactation seems to affect immunomodulatory factors in breast milk. More rigorous research is needed to confirm the emerging evidence. | ca_CA |
