) is associated with an increase in morbimortality.
The
electrophysiological study (EPS) is stablished as the main diagnosis technique in patients with
BBB and
non
-
filiated syncope after the initial clinic
al evaluation
. In
the
cases not diagnosed by
the EPS, the implantation of an implantable loop recorder
(ILR) is recommended to achieve
prolonged electrocardiographic monitoring and it will be able to determine the cause of syncope
during follow
-
up and
to
assess the need
ing
for pacemaker implantation.
Purpose
:
The p
rimary endpoint was t
o determine the diagn
ostic
yield
of the ILR in patients
with syncope and BBB who could not be diagnosed by the EP
S, as well as the analysis of
p
acemaker
implant predictors in the
follow
-
up.
Secondary endpoint was to evaluate long
-
term
mortality
of the ILR strategy.
Material an
d methods
: Retrospective
and
unicentric study that included patients with
BBB and
non
-
filiated syncope. All the patients recieved
an ILR after evaluation by non
-
diagnostic EPS.
Clinical, electrocardiographic, echocardiographic and electrophysiological vari
ables were
analyzed.
Results
: A group of 65 patients
with a
mean
age ± SD
of 73.3 ± 8.9 yea
rs were included in this
study.
ILR allowed the diagnosis of the cause of syncope in 25 patients (38.5% of the
total
patients). The main aetiology
of I
LR diagnosis t
hat explained a
p
acemaker implantation was
p
BAV (n = 20) followed by sinus dysfunction (n = 5)
. A
ny other arrhythmic causes such as
supraventricular paroxysmal tachycardia or ventricular tachycardia
were documented
. In the
univariate analysis,
no significa
nt predictors of p
acemaker implantation were obtained.
A total
of 12
deaths
were observed
(18.5%) without significant differences i
n the group of non
-
diagnostic
ILR
vs
diagnostic
ILR
(n = 8 [20%] vs n = 4, [16%], respectively, p = 0.75). The
Kaplan
-
Meier c
urve did not show significant differences in mortality during the long
-
term
follow
-
up of these patients (Log Rank test, p = 0.56).
Conclusions
: The diagnostic strategy of ILR after
a
non
-
diagnostic EPS in patients with
syncope and BBB
provided
a diagnos
tic yield of
38.
5% and the main
aetiology was p
AVB
. This
protocol allowed
to de
termine the patients that
could
benefit from
a
pacemaker implantation,
thus avoiding the implantation of pacemaker in 61.5% of ILR carriers
. This was a
safe strategy

electrofisiológico (EEF) se ha establecido como la principal técnica diagnóstica en pacientes
con síncope no filiado tras
la evaluación
clínica
inicial y que presentan

E
n los casos no
diagnosticados mediante
el EEF se recomienda el implante de un registrador de eventos
implantable (REI) para conseguir una monitorización electrocardiográfica prolongada y as
í
poder determi
nar en el seguimiento la causa del síncope y valorar la necesidad de implante de
marcapasos (MP).
Objetivos:
El objetivo
principal
del estudio fue d
eterminar el rendimiento diagnóstic
o
del
REI
en pacientes con síncope y BR que no pudieron ser diagnosticado
s mediante el
EEF

í como
el
análisis de predictores de implante de MP en el seguimiento. El objetivo secundario fue estimar
la
seguridad en términos de mortalidad de la estrategia de implante de REI en estos pacientes.
Material y métodos:
Estudio unicén

retrospectivo que incluyó pacientes con síncope no
filiado y
BR a los que se les implantó un REI tras una valoración mediante EEF no diagnóstica
.
Se analizaron variables clínicas, electrocardiográficas, ecocardiográficas y electrofisiológicas

estrategia diagnóstica de RE
I tras un EEF no diagnóstico en pacientes con
síncope y BR presenta un rendimiento diagn
óstico del 38,
5%, siendo la
principal etiología el
BAVp. Este protocolo permite determinar los pacientes que realmente se benefician de implante
de marcapasos
, evitando
por lo tanto, el implante de MP en el 61,5% de los portadores de REI
siendo
ésta
una estrategia segura
en términos de mortalidad total. [-]
