agreement on the usefulness of CRP as a measure of the effectiveness of antibiotic
therapy. Therefore, our objective is to evaluate it
’
s usefulness as a predictive test of
clinical improvemen
t in the follow
-
up of children diagnosed with CAP with
antibiotherapy and to determine which variables are related to their levels during their
follow
-
up.
Material
s
and methods.
Retrospective observational study of historical cohorts,
includes 151 children
admitted with the diagnosis of CAP in the
Hospital
General
University of Castellón between 2016 and 2017.
Results
.
Patients admitted with NAC with antibiotherapy and good clinical course
present a median reduction of CRP with respect to admission of 81.41
% (IQR 73.66
-
87.43%). In 93% of patients, it decreased more than 50%. The presence of pleural
effusion is associate
d with a lower reduction of CRP and it
is associated with longer
duration of fever prior to the antibiotic.
Conclusion.
CRP is an analytica
l parameter predictive of clinical improvement,
although it
’
s use is not justified in patients with CAP who progress favorably. However,
it would be useful if they present a pleural effusion to determine the duration of
intravenous antibiotic therapy. [-]

la PCR como
medida de efectividad de la
antibioterapia
. Por
tanto nuestro objetivo es
evaluar su utilidad como prueba predictiva
de mejoría clínica en el seguimiento de niños di
agnosticados de NAC con
antibioterapia y determinar qué variables se relacionan con sus niveles durante su
seguimiento.
Material y métodos.

151 niños ingresados
con el diagnóstico de
NAC

spital General
Universitario
de Castellón
entre 2016 y 2017
.
Resultados
.
Los
pacientes
ingresados
con
NAC
con
antibioterapia
y
buena
evolución
clínica
presentan
una
mediana
de
reducción
de
la
PCR
r
especto
al
ingreso
de
81.41%
(IQR
73.66
–
87.43%).
En
el
93%
de
los
pacientes
disminuyó
más
de
un
50%.
La
presencia
de
derrame
pleural
se
asocia
a
una
menor
reducción
de
PCR
y
se
relaciona
con
mayor
duración
de
la
fiebre
previa
al
antibiótico.
Conclusión
.
La PCR es un parámetro analítico predictivo de mejoría clínica, no
obstante
no es
tá justificado su uso
en pacientes con NAC
que evolucionan
favorablemente.
Sin embargo, sería útil si
presentan
derrame pleural para determinar
la duración de la antibioterapi
