Perfil clínico y pronóstico tras un ingreso por insuficiencia cardíaca aguda en función del sexo.

Abstract

Introduction : H eart failure (HF) is one of the main causes of hospitalization among people over 65 years old in our country, which constitutes a high health and social care burden. Our objective was to describe epidemiology and morbimortality stratified by gender after t he first year of registering hospitalization for acute heart failure (AHF) in a tertiary hospital without an HF unit. Methods : All patients admitted with HF diagnosis were included from 1 October 2016 to 31 October 2017. This registry was approved by the E thics Committee for Clinical Research of the Hospital Ge neral Universitari de Castelló and the Spanish Medicine Agency. Statistical data were analysed with the statistical program Stata version 13. Results : A total of 313 patients were registered at 31 Oct ober 2017. The mean age was 73 (±12) years. The 41.21% were women and 33% showed a history of ischemic heart disease. Left ventricular ejection fraction was significantly lower among men (43.5 ± 14.3%) than women (52.6 ±1 3.8%), p < 0.01. A total of 86 ev ents were registered (cardiovascular mortality and readmissions for AHF). Figure 1 shows the Kaplan - Meier curve. When these results were stratified by sex, no significant differences were obtained. Conclusions : HF is one of the causes for the high morbimor tality in our e nvironment, with no significant gender differences.

Introducción : L a insuficiencia cardiaca (IC) actualmente constituye una de las principales causas de ingreso en la población mayor de 65 años en nuestro país, generando un elevado coste socio - sanitario. Nuestro objetivo fue describir la epidemiología y morbimortalidad estratificada por sexo tras el primer año de registro de hospitalización por insuficiencia cardiaca aguda (ICA) en un hospital terciario sin una unidad de IC. Métodos : S e incluyeron a todos los pacientes ingresados con el diagnóstico de IC desde octubre de 2016. Este registro fue aprobado por el Comité Ético de Investigación Clínic a (CEIC) de l Hospital General Universitari de Castelló y la Agencia Española de Medicamento. Los datos estadísticos se analizaron con el programa estadístico Stata , versión 13. Resultados : A fecha de octubre de 2017 , se registraron un total de 313 pacientes. La edad media fue de 73 ( ± 12 ) años. El 41 , 21% fueron mujeres. El 33% mostraban antecedentes de cardiopatía isquémica. Se observó una FEVI significativamente más baja en ho mbres (43,5 ± 14,3%) con respecto a las mu jeres (52,6 ± 13,8 %) , p < 0,01 . Los eventos registrados fueron la mortalidad cardiovascular y los reingresos por IC, siendo un total de 86 eventos. La Figura 1 muestra la curva de Kaplan Meier. Cuando estos resultados se estratifican según el sexo, no se o btuvieron diferencias significativas. Conclusiones : L a IC constituye una causa de elevada morbimortalidad en nuestro entorno hospitalario sin que existan diferencias significativas entre ambos sexos.
